Community Nurse Team Leader

Job summary

St Luke's Hospice in Basildon has a fantastic opportunity for a community nurse team leader, on a part-time basis for 22.5 hours per week.

You will manage a team of community nurses ensuring the provision of high standards of care to patients and their families or carers who have a palliative care need.

Main duties of the job

• Providing leadership and guidance to the team in Hospice Community Services.
• Supporting the teams to ensure the delivery of the right care at the right time to individuals with a palliative care need in the community.
• Working, actively supporting and liaising with other health care professionals and agencies.
• Assisting in audit and research projects, particularly within the service, taking a lead on specified projects as appropriate.
• Monitoring and evaluating team performance, providing guidance and support where necessary.
• Fostering a culture of continuous improvement and professional development within the team.

Anyone living in Basildon and Thurrock can call on us 24 hours a day, seven days a week.

Working alongside other healthcare providers, our specialist services are available at the hospice, out in the community or in a persons place of residence. Together, we fulfil people's choices at the end of their life.

We are a charity and the services we provide are free. We rely on voluntary income from local people to deliver the care they deserve.
