Job summary

St Luke's Hospice has a fantastic opportunity for an experienced and devoted community nurse to join our Rapid Access to Discharge Service on a part-time basis, for 22.5 hours per week. The salary for these hours is £19,312.76 - £24,592.68pa depending on experience. This provides a rewarding opportunity for someone who is passionate about delivering high-quality palliative care services.

You will be instrumental in coordinating the safe and timely discharge of palliative patients from hospital to their preferred place of care. Working collaboratively with ICT teams to holistically assess patients in their own homes, to ensure people are able to achieve their wishes at the end of life.

Main duties of the job

All applicants should have the following qualifications, registrations, skills and experience:

  • A relevant nursing degree.
  • Current NMC registration with an active PIN.
  • Fundamentals of Practice Nursing course (General Practice Nurse Foundation Programme) or equivalent experience or qualifications;
  • Demonstrated clinical experience.
  • Awareness of palliative care.
  • Strong interpersonal and communication skills.
  • Excellent organisational skills with the ability to manage complex situations.
  • A commitment to the values and mission of St. Luke's Hospice.

Working alongside other healthcare providers, our specialist services are available at the hospice, out in the community or in a persons place of residence. Together, we fulfil peoples choices at the end of their life. We are a charity and the services we provide are free. We rely on voluntary income from local people to deliver the care they deserve.
