Job summary
This is an exciting time for Calderdale's district nursing teams as we have established 5 community hubs each with its own unique and diverse needs.
We are seeking an enthusiastic staff member with excellent communication skills, who is highly motivated and adaptable in a fast paced and ever changing environment.
Due to the nature of the role it is essential that you hold a full driving license with access to a vehicle suitable for business use.
We have a 23.5 hour vacancy with the Hub 3 District Nursing team. Whilst this will be predominantly Elland and Stainland area, there will be an expectation to work division wide when required.
We would welcome any informal visit to see our hubs in action and meet the teams.

Job responsibilities
- Be able to create a professional nursing culture which is open to change, new ideas, concepts and innovation whilst reflecting organisational strategy and values
- To deliver direct and indirect patient care.
- To participate in the assessment, development, implementation and evaluation of programmes of care. Calderdale and Huddersfield Foundation Trust provide a 24 hour district nursing service and, as such, an element of flexible working is expected to maintain safe, effective, responsive services.
- Calderdale and Huddersfield Foundation Trust acknowledge and support the NMC Code of Professional Conduct (NMC 20002). This code must underpin the practice of community nurses within the PCR at all times and has informed the development of the competencies outlined below

QUALIFICATIONS / TRAINING
Essential
- NMC Registered Nurse in relevant field of practice
- Practice Assessor/Practice Supervisor qualification or willingness to complete
Desirable
- Additional post-registration qualification relevant to field of practice
- Evidence of continuing professional development
- Role Specific Training relevant to specialty of the post
- Teaching Qualification
- Leadership Qualification
KNOWLEDGE, EXPERIENCE & EXPERTISE
Essential
- Knowledge and skill to plan, deliver, and evaluate high quality, evidence-based nursing care to a group of patients within the speciality of the post
- Experience of working with patients, and those that matter to them, from a range of diverse cultures and backgrounds
- Experience of working as part of a team, with the ability to prioritise own workload
- Knowledge and skill in infection prevention and control principles and procedures
- Knowledge and skill in safeguarding principles and procedures
- IT literacy and ability to use NHS systems
- Competence in speciality or role specific extended skills required of the post or willingness to undertake necessary training.
Desirable
- Clinical experience in the specialty of the post
- Skills in research and audit methodology and application of findings to practice
- Evidence of introducing change and innovation into nursing practice
