Job summary
We are here for anyone in our community who is facing a life-limiting illness. Using our full breadth of service, we create individual care journeys, which start at diagnosis and continue with bereavement support for family, carers and children. Working in the Keynsham area. This is a Full Time position, working 37.5 hours over 5 days.
Main duties of the job
The successful candidate will work around the Keynsham area alongside existing members of a very supportive Community Palliative Care Team and will contribute to the delivery of palliative care by providing expertise in decision making, advance care planning & holistic symptom control.

COMMUNITY STAFF NURSE | Band 5 £29,970 - £36,483 per annum

Person Specification
Qualifications
Essential
- * First Level Nurse with relevant post basic qualification
- * UK driving licence with access to a vehicle
Desirable
- * ENB 998 or equivalent experience in teaching
Experience
Essential
- * Clinical experience at Band 5 level or equivalent
- * Excellent communication/interpersonal skills related to complex patient care
- * Can demonstrate experience working alongside a multi-disciplinary team
- * Ongoing evidence of professional development
- * Personal attributes that reflect care, compassion and willingness to support patients and their families through their end of life journey
Desirable
- * Clinical experience in a relevant specialty with clinical assessment and analytical skills
- * Demonstrates evidence of palliative care knowledge
- * Research skills
- * Experience of working within the community/relevant qualification
- * Experience working independently and manage own caseload
