We are looking for a dedicated and dynamic Community Support Worker to join our urgent community response and intermediate care team working in Scarborough and Ryedale community services.
You will work alongside Physiotherapists, Occupational therapists and Nurses to support carrying out rehabilitation programme's and functional activities to support patients with their rehabilitation in the community.
This post involves working with patients with a wide variety of pathologies and medical conditions, who require a period of support to help improve their mobility and independence.
You will receive regular supervision and opportunities for development along with a robust induction package.
Main duties of the job
Day to day, the role includes the following care carried out in the patient's own home under supervision/delegation of therapists/registered nurses:
- Patient care following a fall
- Moving & handling of patients
- Vital observations
- End of life care and urgent support for people living with frailty
- Recognise and escalate issues of concern and/or safety
- Assisting with activities of daily living, e.g., personal care tasks, meal preparation following injury/illness to prevent hospital admission
- Urgent equipment provision & instruction in use

Skills and Competencies
Essential
- Able to demonstrate excellent inter-personal communication skills
- Able to plan and prioritise effectively and efficiently
Experience
Essential
- Experience of working with or providing support to the general public
- Experience of using a range of MS Office applications e.g. Word, Excel, Outlook, Teams etc
Desirable
- Experience of working within a healthcare environment or service
Qualifications and Knowledge
Essential
- NVQ III or equivalent
- Working knowledge of mental and Physical health issues
