Job summary

An exciting opportunity has arisen within The Early Intervention in Psychosis Service (EIP South) of Nottinghamshire Healthcare Foundation NHS Trust for a passionate and highly motivated Community Support Worker(Band 3) to join our team.

The EIP service is delivered through a multidisciplinary team of practitioners who provide comprehensive assessments and evidence-based interventions for people aged 14-65 who are experiencing a First Episode Psychosis (FEP) or an At Risk Mental State (ARMS) for the development of psychosis.

We are looking for applicants with a keen interest in working with the client groups served by EIP and a desire to effectively support their families, to join a dedicated, enthusiastic and supportive team.

A good understanding of the principles, ethos, assessments and interventions associated with working within an Early Intervention in Psychosis team will be key. We provide additional training and support to contribute to the successful delivery of a high quality NICE concordant service, including Behavioural Family Therapy (BFT).

The EIP team is a community-based service which operates Monday-Friday 9am-5pm.

Main duties of the job

To support and contribute to the timely assessment of service users within two weeks of being referred to the EIP service.To hold a personal caseload under the supervision of qualified members of the MDTTo contribute and input into comprehensive risk assessments for service users, supporting care coordinators with their initial formulation of riskAppropriate professional standards are met.National benchmarking will demonstrate improvements.Managed and monitored via supervision and appraisal structuresContacts and clinical activities expected and the development of a risk management plan.To contribute to the review and updating of risk management plans as necessary and appropriate for service users on their personal caseload.To support the identification of co-morbid symptoms which may be present e.g substance misuse, anxiety, depression, trauma etc and contribute to intervening with these as directed and supervised by qualified members of the MDT.To support EIP Physical Health Practitioners in ensuring service users receive regular physical health checks at assessment, 3 months, 6 months and annually thereafter in line with national standards.The post holder will be accountable and report to the EIP team manager.To hold a caseload of patients under the supervision of a qualified member of staff.

Job responsibilities

To support and contribute to the timely assessment of service users within two weeks of being referred to the EIP service.To hold a personal caseload under the supervision of qualified members of the MDTTo contribute and input into comprehensive risk assessments for service users, supporting care coordinators with their initial formulation of riskAppropriate professional standards are met.National benchmarking will demonstrate improvements.Managed and monitored via supervision and appraisal structuresContacts and clinical activities expected and the development of a risk management plan.To contribute to the review and updating of risk management plans as necessary and appropriate for service users on their personal caseload.To support the identification of co-morbid symptoms which may be present e.g substance misuse, anxiety, depression, trauma etc and contribute to intervening with these as directed and supervised by qualified members of the MDT.To support EIP Physical Health Practitioners in ensuring service users receive regular physical health checks at assessment, 3 months, 6 months and annually thereafter in line with national standards.To support EIP Physical Health Practitioners in the delivery of interventions focused on improving and promoting good physical health, both on an individual and group basis.To ensure that service users, carers and family members receive clear and accurate information on:o How to access the serviceo Confidentialityo Access to recordso Their own assessments and care planso What can be expected from the serviceo Supportive community resourceso Complaints and compliments procedureso Accessing advocacy servicesTo support service users to develop clear recovery focused behavioural goals and support service users to attain these during their time in the EIP service.To contribute to and support the delivery evidence-based interventions including Family Interventions and CBT-informed interventions targeting key maintaining factors related to the service users presentation as directed by qualified members of the MDT.To support the development, contribute to and deliver socially inclusive groups and other interventions in conjunction with qualified members of the MDT in the service.within the role are monitored within supervision and reviewed via, clinical notes and completed snomed-ct recording of clinical activities.Mandatory training and role-specific training completed in line with Trust and service standards.To regularly employ standard outcome measures (including HoNOS, QPR & DIALOG) to monitor the efficacy of treatment plans and service user recovery and support the adjustment of care plans accordingly.To support effective discharge planning in collaboration with care coordinators, CBTp Therapists, NMPs and the service user.To support the development of effective relapse prevention strategies and plans in collaboration with service users and their families.To sign-post to various resources, opportunities and activities within the Trust and in communities to promote choice and informed decision making.To encourage service users to attend appointments and activities aligned to their recovery goals in the first instance and where necessary to accompany service users to these whilst continuing to promote independence in the achievement of goals.To establish supportive and respectful relationships with people using mental health services, their family members and carers.To work with the wider MDT and feedback patient care.Documentation of all clinical activities using the Trust Electronic Patient Record System and ensuring accurate recording and reporting if these using the appropriate snomed-ct codes.To keep up to date with the National Standards and policies related to EIP services.Promote the EIP service ethos and culture of recovery, empowerment and informed decision-making.To actively support and promote a culture of learning and continuous development within the EIP service by sharing knowledge, skills and appropriate attitudes in line with the Trust values and behaviours.To engage in all service and role-specific training in addition to Trust mandatory training to support competent delivery of the role of a community support worker within EIP.To liaise and link with the full range of staff including: General Practitioners, Consultant Psychiatrists, Inpatient care, Voluntary Agencies, Social Services, Statutory AgenciesTo undertake any other duties which may reasonably be regarded as within the nature of the duties and responsibilities/grade of the post as defined DimensionsThe post holder will be accountable and report to the EIP team manager.To hold a caseload of patients under the supervision of a qualified member of staff.

Please note that this post does not meet the pay level required for a Skilled worker visa.

Successful applicants with no prior NHS experience would normally be placed at the bottom of the band in line with Agenda for Change.
