Job summary
We have an exciting opportunity for a Consultant Psychiatrist to join our Old Age CMHT Team.
This is a full-time post in Bromley Borough in the Adult Community MH directorate, which is one of five directorates within Oxleas NHS Foundation Trust. The integrated directorate provides mental health and adult community services in the Borough of Bromley.
Mental health provision in the community to residents above the age of 65 is the responsibility of the Older People's Community Mental Health Team. The team is fully integrated with Bromley social services and provides health and social care for older adults. The post holder will be based at Bridgeways Day Hospital, working alongside the other consultant who provides part time input to the team.
Main duties of the job
Towork closely with GPs to ensure early identification of patients with mental health needs and facilitate timely referrals. Assessments will be provided by a multi-disciplinary team, ensuring carers and family are involved at the initial assessment and in care planning.
Through early diagnosis of mental disorders the delivery of evidence based therapeutic initial interventions (pharmacological and psychological) will be facilitated in a timely fashion.
Support and information delivered in a person-centred way after the diagnosis to ensure the person with dementia and their carer are adjusting to the diagnosis and its consequences and are engaging in personal care plans and understand how to access other assistance.
Continuity of care across the pathway and integration at patient level with other care providers
Delivery of care in a considered, timely and co-ordinated manner
This integrated approach to the care of the elderly and the support of their carers, will be provided in partnership with local healthcare, social care and voluntary/ independent organisations.
Additional HTT services are provided through a dedicated OPMH HTT team based at Bexleyheath Centre that covers all 3 boroughs (Greenwich, Bexley and Bromley)

Job responsibilities
Assessment and management of patients in the community with mental health needs who meet the teams inclusion criteria.
To provide a detailed formulation of needs
Joint working, support and advice on the management of appropriate patients to other members of the Community Mental Health Team as well as to colleagues in Social Services
To participate in care planning (or equivalent) meetings
To liaise with other parts of the service (especially the wards and Hub when urgent assessments are needed) . To also liaise with GPs and non NHS organisations when indicated
To cover for colleagues during planned absence and short periods of sick leave
Supervision of junior medical staff
To support with medical leadership and senior clinical support to the multi- disciplinary team, and more broadly across the Directorate
To undertake more complex capacity assessments and apply the Mental Capacity Act and Mental Health Act appropriately
To deliver pharmacological treatment in accordance with NICE guidance.
