An exciting opportunity has become available for a Band 5 Registered Children's Nurse to work within the Greenwich and Bexley CYP Specialist Inclusion nursing team. This is an excellent opportunity for professional growth and development.
The Community Children's Nursing team and the CYP Inclusion Specialist nursing team are part of an integrated network of services for children in Bexley and Greenwich providing high quality specialist child centred care. The team of specialist and generic children's community nurses provides skilled, comprehensive nursing care to children and families with nursing needs in their own homes, special schools, or community settings.
The successful candidate should be to provide a high standard of evidenced based nursing care for children and young people with the following conditions: acute and long-term conditions, children with disabilities and complex conditions including those requiring palliative and end of life care. To provide clear visible leadership and professional clinical expertise to the junior team members, ensuring every aspect of the child and families experience is delivered in a safe, efficient, and evidence-based manner.
This is a post suitable for a Children's Community Nurse who should have an interest in or a willingness to develop and extend specialised skills and knowledge.
Main duties of the job
The CYP Inclusion Specialist School nursing team provides nursing support for children in Bexley and Greenwich providing high quality specialist child centred care. The team of specialist and generic children's community nurses provides skilled, comprehensive nursing care to children and families with nursing needs in their own homes or community settings.
The CYP Inclusion Specialist Nursing Team, providing nursing care to children within the school environment primarily, the child's home, or other community setting; ensuring the delivery of evidenced based to children and their families. This is a post suitable for a Children's Nurse who has an interest in or a willingness to develop and extend skills and knowledge in specialist community nursing for children.
To work in partnership with education teams, other community based statutory, voluntary organisations to deliver seamless services for children and their family.
The CYP Specialist School Nursing team has a competency framework to support the process of induction and professional development for the Children's Community Nurse. After completion of this framework the post holder will share a caseload with a senior member of the nursing team and be able to work without direct supervision

Job responsibilities
Key Task and Responsibilities
- Ensuring the Community Childrens Nursing Service (0-19 years) provides a high quality, evidence based locally appropriate service to families.
- To work as part of the team providing nursing assessments and interventions to children.
- To work in partnership with children and families to implement individualised care plans to meet medical and nursing needs.
- To be responsible for assessing, planning, implementing and evaluating individual childrens care plans.
- To refer children to acute and other services as appropriate and required.
- To work collaboratively with other agencies to deliver clinical care pathways for children and families.
- To record accurately data as required by the service.
- To work as part of the Community Childrens Nursing Team to provide support in an area of special interest from acute to community settings.
- To provide support with specialist training to meet the childs needs. This may be to the child, their family or other professional working with the child in a variety of community settings.
- To attend specialist clinics with families to work towards multi professional assessment and care planning with the child and family.
- To work as part of a multidisciplinary team to ensure a smooth discharge from acute care to community settings as well as smooth transitions into Adult Services.
- To participate in CAF/TAC, becoming a key worker for families where appropriate.
Management responsibilities
- To provide and maintain high standards of nursing care at all times.
- To meet regularly with supervisor to discuss any concerns or significant changes in client care.
- To work under the direction of a Senior Nurse to organise time and daily caseload management.
- To ensure a risk assessment is undertaken before visiting any family and that attention to lone working guidance is observed.
- Ensure the effective and efficient use of resources.
- To act as a Champion for a identified area, attend relevant meetings and ensure team members up to date with relevant policies and guidelines.
Leadership
- To act as role model to members of the nursing team and motivate them to work towards high standard of practice.
- To prioritise workload to ensure effective time management. strategies are embedded within the nursing team, aware of competing pressures to ensure best practice.
- To exercise good personal time management, punctuality and consistent reliable attendance.
- To act as a mentor to students, under the supervision of a more senior colleague, helping to develop and support the students through their placements, completing documentation where necessary.
Clinical
- To assess, plan and evaluate care of individual children within the family context and refer to appropriate services.
- To support the delivery of care and interventions to meet the needs of individual children/ young people and families.
- To perform clinical procedures within sphere of competence.
- To empower families to care for their acutely ill child.
- Work across professional and organisational boundaries and facilitate partnership working.
- To identify limits of own knowledge and skills and seek advice as and when necessary from team members or specialist practitioners or other members of the multi-professional team.
- To be responsible for the reporting of incidents, comments and complaints from children, their family and other professionals involved with the child to the Team Manager.
Research
- Work as part of the team to establish best practice protocols and guidelines for evidence-based practice.
- Participate in clinical nursing audit.
- To collect data as requested to inform practice and service development.
- To utilise theory, evidence-based literature and research to support evidence based practice in individual work and work with other team members.
- To undertake appropriate research and provide research advice to other staff undertaking research.
- To undertake projects, audit and research in collaboration with the Community Childrens Nursing Team.
Communication
- Work closely with professionals in education and the community and the acute setting to ensure a robust and safe nursing care for children and young people and their families.
- Support the clinical care pathway with hospital staff on all matters that affect the management and readmitting of child into hospital.
- Contribute to data collection and reporting systems.
