Job summary
We are looking for a qualified Practice Nurse who has completed a Diabetes Diploma, or equivalent, to provide clinical nursing services and health promotion advice to patients registered with the Practice, who will be central to good patient care and outcomes for those with diabetes.
Main duties of the job
Working over two full days, you will run structured diabetes clinics, assessing, planning, evaluating and implementing personalised treatment plans and supporting patients to manage their diabetes in a safe and effective way.

We are a friendly, high achieving, 6 Partner GP Training Practice in the centre of Emsworth, Hampshire, looking to recruit a part-time Diabetes Nurse, to play a key role in delivering specialist services to patients living with diabetes.
We have growing population of 15,000 registered patients. Our clinical team is made up of GPs, 2 Nurse Practitioners and a Nursing Team of Practice Nurses, HCAs and Phlebotomists.
We are part of the Hayling Island and Emsworth Primary Care Netowrk.
Job responsibilities
Assess patient diabetes care needs through structured reviews, offering expert advice in all aspects of complex diabetes management, delivering personalised treatment plans, including insulin initiation.
Review medication effectiveness, ensuring it remains appropriate for patient needs.
Offer supportive care to patients, particularly those newly diagnosed or who are facing diabetes related complications.
Collaborate with patients to encourage adherence to prescribed treatments.
Maintain effective communication with patients, their families and other healthcare professionals.
Promote safe and independent management of diabetes to patients and their families.
Ensure accurate record-keeping.
Work closely with GPs and the Nursing Team to help ensure that that diabetes care within the practice is delivered efficiently and professionally.
Serve as a valuable resource for specialist clinical advice and expertise.
Keep up-to-date with relevant protocols and knowledge.
Person Specification
Qualifications
Essential
- Registered Nurse (RN) qualification or equivalent nursing degree.
- Specialist Diploma in Diabetes.
Experience
Essential
- Proven experience in patient care.
- Familiarity with recognised diabetic processes.
- Excellent communication skills, both verbal and written.
- Ability to work effectively as part of a multidisciplinary team.
- Strong organisational skills with attention to detail.
- Empathy and compassion towards patients' needs.
Desirable
- Experience of working in Primary Care
- Experience of working in General Practice.
Computer Skills
Essential
- Competent in using a computerised patient records system.
Desirable
- Familiar with System1 computerised system.
