Job summary

An exciting opportunity has arisen for a Band 6 Diabetes Specialist Nurse position at Nevill Hall Hospital Diabetes centre, to join our dynamic Diabetes Specialist Nursing Team. We are looking for a motivated, enthusiastic and experienced nurse to join our Diabetes team. We value personal qualities such as compassion, kindness and the willingness to be flexible in order to meet the needs of the service. We are keen to foster support and provide training opportunities for personal development.

Main duties of the job

  • Act as advocate for patients, healthcare staff and the healthcare department.
  • To provide professional, clinical, and operational leadership and support to healthcare professionals.
  • Responsibility for the formulation and management of clinical action plans.
  • Responsibility for clinical audit.
  • To provide leadership and support within primary/secondary care with the delivery of diabetes management.
  • To maintain and promote accurate record keeping and documentation following national and professional standards and local protocols and policies.
  • To collect and input clinical data to clinically based computer systems: to access and utilise all available clinical information to support care planning and patient management.
  • To ensure effective and safe medicines management processes are adhered to.
  • To support patients with insulin pumps and continuous monitoring technology.
  • To plan and provide education for patients with diabetes, to enable them to self-manage their condition.
  • Ensure awareness of sources of support and guidance, and provide information in an acceptable format to all patients, recognising any difficulties and referring where appropriate.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
