Job summary
An exciting opportunity has arisen for an enthusiastic, professional, and experienced Diabetes Specialist Nurse to join our excellent Nursing Team at Brockworth Surgery. The ideal candidate will be experienced in diabetes management, willing to learn, able to take on responsibilities and have a good team spirit. We are looking for the candidate to work around up to 20 hours or more per week.

Main duties of the job
The candidate will need experience managing diabetic patients whether that it is a hospital/community setting or working within general practice.

Brockworth Surgery is a practice based in Gloucestershire caring for over 11,000 patients. We are a teaching practice and our team consists of 4 GP Partners, 6 salaried GPs, 1 Lead Practice Nurse, 2 part-time Practice Nurses, 1 Nurse Associate, 3 Healthcare Assistants and a Phlebotomist. We also work within a multidisciplinary team of Mental health practitioners, Pharmacists, Care Co-ordinators and Social Prescribers. We have a very good team ethic at the practice and all work together to ensure high quality care is given to our patients.
Job responsibilities
Specialist Diabetes Nurse or Diabetes Practice Nurse who will work within the nursing team providing care to our patient population
They will be expected to:
- Run Diabetes clinics, reviewing bloods, medication management, lifestyle promotion, referrals to diabetes services
- Work alongside the diabetes practice nurse
- Have access to PCN specialist diabetes consultant clinic
