Job summary

Ward Two is currently looking to recruit a proactive and patient centred Discharge Coordinator. As a ward we prides ourselves on our effective team work. As a discharge Coordinator on Ward Two, you will be required to interact closely with all members of the Multi-Disciplinary Team and other external health and social care services in order to facilitate safe and effective discharges for our respiratory patients. Base Location: Royal Bournemouth Hospital Interview Date: TBC

Person Specification

Qualifications

Essential

  • Numeracy and Literacy-GCSE / A level or equivalent standard

Desirable

  • NVQ level 3: Health/ Social care management or equivalent.

Experience

Essential

  • Understanding of patient flow and supporting complex discharge processes in an acute ward area.
  • Experience of caring for patients with acute and long term conditions in a hospital setting
  • Demonstrable clinical experience

Desirable

  • Experience of supporting others with the process of planning patient discharge requiring CHC and Social services involvement.
  • Experience of patient flow and discharge planning systems

Technical Skills Competencies

Essential

  • Basic IT skills
  • Assessment, planning and organisational skills.

Desirable

  • Awareness of audits
  • Good knowledge of NHS computer systems, e.g Ecamis, EPR

Knowledge

Essential

  • Demonstrated knowledge of both simple and complex discharge form an acute ward, including mental capacity act
  • Understanding of National Frameworks, CHC, Delayed Discharge Act and reimbursement
  • Understanding of MDT assessment and treatment for vulnerable patients within acute care and community settings.

Desirable

  • Knowledge of nation guidance and target relating to discharge
