Job summary
We have an exciting opportunity for a Discharge Manager within our Frailty assessment Unit (FAU). We are looking for an individual who is passionate about ensuring safe, effective and timely discharges.
This senior position offers the opportunity for the successful candidate to be a key member of the multidisciplinary team, interfacing between the hospital and community, providing expert advice on discharge planning and admission avoidance through holistic assessments and collaborative working with partner organisations.
Main duties of the job
The post holder will maintain and develop the necessary knowledge of local and national discharge systems, processes and legislation relevant to the post to enable best practice and pro-active discharge effectiveness.
Possesses excellent communication skills, receiving and disseminating complex, sensitive information.
Communicate effectively across organisational and professional boundaries.
Manage situations that may be complex in nature. Make sound clinical decisions based on the situation to ensure the best outcome for the patient.
Support and contribute to the development and maintenance of systems monitoring, evaluating audit for the service to meet Trust requirements.
Co-ordinate and integrate care across health and Social care, preventing duplication, fragmentation and delay as patients move between care settings.
Make direct referrals as necessary following agreed protocols and pathways of care, ensuring the medical records are updated.
Assess risks which affect the health and safety of individuals
Prioritise and manage the implementation of change in work activities, taking a flexible approach to service delivery.
Contribute to the development of protocol/pathways and procedures to support effective service change and delivery across health and social care.
To be aware of financial and physical resources, ensuring values for money and best use of resources.
Deputise for Senior Discharge Manager and Team manager when required.

Qualifications and training
Desirable
- Diploma in Nursing orsimilar AHP qualification
Clinical Skills and Experience
Essential
- Minimum 2 years experience working within health or social care at a senior level
- Experience of discharge planning in an acute care setting
- Excellent communication skills both verbal and written
- Ability to advocate as patient advocate
Desirable
- Experience working in the community
- Experience of working within both acute and primary care
Governance
Essential
- Awareness of professional issues and recent developments in the NHS and Primary care
- Ability to support the implementation of local and national agendas for health under guidance
- A good understanding of the discharge planning process
