Job summary

An exciting opportunity has arisen within the Discharge Team. You will work as part of team co-ordinating referrals received from our wards and sending to the appropriate service following a validation process. You will helping to improve patient flow primarily working at Poole Hospital. The role will incorporate cross site working and cover with Bournemouth Hospital.

You will be working closely with our Partners and building good working relationships.

The role is full time and cover a 7-day working pattern.

Base Location: Bournemouth Hospital

Experience

Essential

  • Ability to collect and produce reports
  • Ability to type reports, letters, emails
  • Be able undertake task at short notice

Qualifications

Essential

  • Word to intermediate standard
  • Database management

Technical Skills

Essential

  • Data analysis & Reports using Excel and hospital in house systems.
  • IT applications
  • Referrals, systems and processes

Desirable

  • Basic knowledge of medical and Delayed Transfers of Care & D2A terminology
  • Knowledge of Hospital systems such as Health of the Ward & Electronic Patient Record (EPR)

Knowledge

Essential

  • Work as part of a team
  • Be able to communicate effectively with other disciplines outside the Trust
  • Able to work on own initiative
