Main duties of the job
An exciting opportunity has arisen to join our Community Nursing team as a District Nurse team leader.
Rated OUTSTANDING for CARE by the CQC, you will be leading a team of highly skilled and compassionate nurses, nurse associates and health care assistants.
You will manage a caseload of patients, attached to 1 or 2 GP surgeries, including case management of patients identified as being at risk of hospital admissions. Provision of skilled, professional holistic assessment of health needs, relevant nursing care and health information to members of the adult community is required.
The Team Leader works in partnership with the primary health care team, social services, private and voluntary sector, addressing health promotion priorities both locally and nationally.
You will need to have a District Nursing qualification and experience of managing a team. You will be expected to manage patients with a range of long-term conditions and multi pathologies.
You will act as a role model, teaching and supporting staff and students on placement in the community, so will need to be a trained Practice Assessor.
You will also manage team members, developing their skills and providing feedback through 1-1 meetings and appraisal and managing sickness and capability as required. You will also be involved in audit to ensure care remains at a high quality and evidence based.
You will need to be a car driver and have access to a vehicle.

- Registered Level 1
- District Nurse Qualification or equivalent
- Evidence of recent post registration, professional and portfolio development

- Community experience of assessment and delivery of care to people with complex needs
- Negotiating and working across organisational boundaries

- Understanding of national policy governing the delivery of adults and older peoples services

- Clinical Supervision and appraisal
- An up to date working knowledge of :-
- Acute, chronic and terminal illness, wound management, care of the elderly, care of the post op patient, drug interactions, aetiology and physiology of pain, pain assessment, and strategies for effective pain and associated symptom control.
- An understanding of the implications of cultural difference for service delivery
- Knowledge and understanding of audit and research
