DistrictNurse Service Lead

An exciting opportunity has become available for a band 8A District Nursing Service Lead Post.

This post for the District Nurse Service Leadership Team is an exciting opportunity for an individual to work alongside the existing District Nurse Service Lead to lead, manage and develop the Community District Nurse service.

We are seeking a highly motivated District Nurse with the aptitude, motivation, and leadership skills, to drive local and National changes and integration within community nursing. You will lead DN community nursing teams, working closely with the acute hospital, GPs, and Practice Nurses to ensure high quality care for the local population to ensure effective, efficient, safe, and high-quality nursing care for patients and carers.

Community Services are currently undergoing significant change in the delivery of community services to continuously improve patient care and outcomes, meet workforce demands, prevent inappropriate hospital admissions, and facilitate early discharge. The successful candidate will work with their Nursing Team leads to maintain capacity across community services to help achieve this. The post holder will work closely with the Community Nursing Services Lead to ensure the service is working in line with the service level agreements, Performance is achieved in terms of, KPIs, Finance, HR, Risk and Governance.

Main duties of the job

The post holder will have extensive experience and knowledge in managing clinical governance including incidents, risk registers and audits.

The post holder will leadtheDistrictNursing Teams, being the first point of contact both from an internal and external perspective. You will be responsible for a wide range of general and operational management issues aimed at managing health economy flow and supporting "home first". This will be achieved through close liaison with General Practice, CommunityLeadfor AHP and NursingServices,NursePractitioners, Allied Health Professionals, ICTs, etc.

Person Specification

Education & Qualifications

Essential

  • First level Registered Nurse
  • District Nursing Specialist Practitioner
  • Masters level education or equivalent knowledge / experience
  • Evidence of formal Leadership / Managerial Training and Development
  • Demonstrates a comprehensive understanding of clinical governance
  • Awareness of national changes in Health Policy and NHS reforms

Desirable

  • Evidence of commitment to life-long academic learning

Knowledge and Training

Essential

  • Evidence of leadership / management qualities
  • Demonstrates up to date evidence based clinical trends in chronic disease management
  • Computer literate - ability to use software programmes designed to maximise the contribution to the post
  • Demonstrable contribution to practice developments in primary care / disease management

Desirable

  • Specialist knowledge and skills relating to complex chronic disease
  • Up to date knowledge of current Government and Local imperatives and how these translate into local practice
