The Enhanced Practice Nurse (EPN) plays a pivotal role in delivering comprehensive healthcare services, particularly under the Enhanced Health in Care Homes (EHCH) framework. Working within a multidisciplinary team, the EPN will provide advanced nursing care to patients in care home settings, ensuring high-quality, evidence-based care. This role includes patient assessment, care planning, health promotion, and contributing to quality improvement initiatives.

Main duties of the job

Comprehensive Patient Assessment and Care Planning

- Conduct thorough assessments of patients' physical and mental health needs.

- Develop, implement, and evaluate individualized care plans in collaboration with patients, families, and the multidisciplinary team.

Clinical Decision-Making

- Utilise clinical expertise to analyze complex health conditions.

- Apply evidence-based interventions and specialized equipment to support clinical decisions.

Enhanced Clinical Care Delivery

- Provide advanced nursing care in dynamic and challenging environments.

- Adapt to evolving healthcare models, incorporating innovative practices and technologies.

Patient and Family Education

- Educate patients and their families on managing health conditions effectively.

Quality Improvement and Research

- Implement changes based on audit findings and contribute to the development of practice protocols and guidelines.

Professional Boundaries and Delegation

- Recognise the limits of personal competence and refer patients appropriately.

- Delegate tasks to other team members, ensuring accountability and supervision.

Trent Care Network (TCN) consists of 5 GP surgeries in Saxilby, Gainsborough and Hibaldstow. We cover a population size of 40,198 patients with a range of diversity.

Person SpecificationQualifications:

- Degree in Nursing or completion of a Registered Nursing Degree Apprenticeship (RNDA).

- Registered with the Nursing and Midwifery Council (NMC).

- Postgraduate qualification at Level 7 or above in a relevant field (e.g., wound care, diabetes management, respiratory care, cardiovascular disease, dementia care, women's health, public health, or population health management).

Experience:

- Demonstrated experience in managing complex patient care within a multidisciplinary team.

- Experience in supervising and mentoring.

Qualifications

Essential
