Job summary
Full time, permanent position.
We are seeking a highly skilled and enthusiastic Falls Lead Practitioner who will play a pivotal role in the development and delivery of the falls service, aimed at identifying individuals at risk of falling and implementing strategies to prevent falls and fractures.
As part of the corporate team you will be committed to ensuring falls prevention and patient safety is at the heart of everything we do. Reducing length of stay and supporting keeping patients out of hospital and in their preferred place of care.
You will work closely with a skill mixed specialist team to provide holistic care, education, and support to patients and their families. Your expertise will help improve patient outcomes, enhance quality of life and the patient experience.
Main duties of the job
The Falls Lead Practitioner will be responsible for delivering training and education on falls prevention, identifying training needs among staff, and fostering a proactive safety culture within the organisation.
They will model professional standards and engage staff in a supportive environment, promoting compassionate engagement and psychological safety6.
Additionally, the post holder will apply Quality Improvement methodology to support Trust-wide projects with a clear focus on falls prevention.
The role requires an experienced Allied Health Professional (AHP) or Registered Nurse (RN) who will support the ongoing management of patient falls, implementing medium and long-term strategies to reduce falls, conducting audits, and analysing falls data.

Experience

- Extensive post qualifying experience of working autonomously at a specialist level delivering effective patient focused care to vulnerable adults with complex presentations.
- Experience of audit, research and evidenced based practice.
- Experience in delivering change.
- Extensive experience of treating people with a range of physical needs e.g., poor posture, poor upper or lower limb function, balance problems, visual impairment.
- Supports self-care, self-management and enabling independence
- Previous experience of student supervision and clinical education at undergraduate level.
- Well-developed professional leadership skills
Desirable
- Demonstrates ability to work independently/autonomously without supervision within the community setting
- Demonstrates interagency and partnership working
- Demonstrates masters Level academic ability
