Job responsibilities

JOB PURPOSE

To implement and evaluate individual treatment plans for patients for a variety of health needs.

To work as part of the Practice nurse team providing general practice nursing care to patients.

Work autonomously following local and national guidance.

1. To work autonomously with patients following local and national guidance.
2. Assess, develop, and implement nursing care programmes for patients with a variety of health needs.
3. Teaching and educating patients and their families by offering information, which enables them to understand their illness and its progress.
4. Manage medication treatments as required following Practice protocols.
5. Refer patients directly to other specialists in other professions, as appropriate.
6. Act as a resource, providing specialist knowledge, gained through clinical experience to other members of the multidisciplinary team including GPs, registrar students, medical students, nursing students, district nurse students.
7. Participate in clinical audits/research projects and implement changes as required.
8. Participate in the development and updating of Practice protocols/guidelines and procedures within the Practice.
9. To undertake any aspect of Practice Nursing duties including (but not limited to):
1. Taking cervical smears/triple swabs and interpreting and acting on swab results.
2. Travel health advice and vaccination.
3. Administer injections under individual patient prescription or Patient Group Directives.
4. Ear syringing
5. Dressings and wound care, to include removal of sutures and clips
6. Contraception and female health
7. Maintain safe storage, rotation and disposal of vaccines and drugs.
8. Apply infection control measures within the Practice according to local and national guidelines.
9. Keep up to date with safeguarding policies and procedures.
10. Deal with complex and sensitive information with patients, relatives, and colleagues.
11. Recognise psychological needs of patients presenting with depression or suicidal tendencies.
12. Competent in anaphylaxis and resuscitation procedures.
13. Promote the health and safety of patients and work colleagues by identifying hazards and assessing clinical risks within the workplace and ensure that all practice policies are adhered to.
14. Responsible for entering details of clinical consultations accurately on computer records, always maintaining patient confidentiality.
15. To undertake any other duties which are commensurate with the level of and responsibility of the post.
16. To always provide an exemplary standard of professional conduct and nursing care in accordance with the NMC.
17. To maintain personal and professional development, ensuring regular updates are undertaken.
18. A good attitude and positive action towards Equality Diversity & Inclusion (ED&I)
10. To facilitate the development of individual staff, including undertaking mentor sessions with junior staff, as required.
11. Support the nurse team to provide a suitable learning environment that is conducive to the acquisition of further skills and knowledge for all staff, for formal and informal personal and professional development.
