This is a full time role but part time would be considered, please state on your application.

Act as 'front door' for access to housebound patients by proactively scanning duty lists for appropriate same day home visits to be able to provide high quality compassionate care and assistance to patients in their own homes. Ensuring housebound lists are maintained and up to date. Co-ordinate visits to ensure all aspects of Primary and PCN contracts are delivered to patients in their own homes e.g. (QOF) (LES) and (IIF). Tackle health inequality by providing access to Year of Care, NHS Health checks etc to housebound patients. To improve overall team performance through better use of admin systems and co-ordination. This position consists of 3 elements and successful performance in these domains will streamline the duty doctor list allowing resources to be allocated appropriately, free time for the clinical team to focus on patients with higher level need. It will increase patient satisfaction through getting the right service, to the right patient, at the right time and reducing the risk of clinical error through ensuring admin systems are up to date and address health inequality.

The 3 elements we are looking for are direct patient care, administration and social prescribing. An outline of tasks for each element is provided below, this is not exhaustive.

This role will require you to visit people in their own homes. You will require a driving licence and have access to your own car.

Main duties of the job

Patient Care:

  • First part Year of Care including diabetic foot check; inhaler technique; ACT/CAT; peak flow; BP/ABPM
  • NHS Health Check/New patient check
  • Health promotion activity-smoking cessation; AUDIT-C; GPPAQ; diet
  • ECG/Venepuncture
  • Vaccinations under PSD (flu/shingles/pneumo)
  • Urinalysis
  • Raise concerns with senior staff/GP in a timely way, referring to safeguarding where appropriate
  • Develop awareness of locally/nationally available services and signpost patients to them
  • Review "light touch" patients referring any concerns to senior staff
  • Assist with audit/research projects

Admin:

  • Check daily duty lists, make initial contact with patients known, or suitable for the Home Visiting Team
  • Allocate to routine home visits, Acute Clinical Practitioner (ACP) home visit or return to GP duty list
  • Follow up referrals made to social care/CRRT/secondary care
  • Check housebound EHCP/DNACPR/YoC lists monthly and allocate visits accordingly
  • Regularly review and update housebound patient list, ensuring that coding is correct
  • Monitor housebound patients, QOF, LES and IIF ACHIEVEMENT
  • Liaise with care home coordinators
  • Liaise with other teams for service including physio, podiatry, dieticians etc.

SPLW:

  • Provide support to patients with non-complex needs, referring to senior staff as necessary
  • Health promotion activity
  • Signpost to specialist services for example drug, alcohol, dementia, local authority and financial as necessary

Our Primary Care Network is led by one of our GP Partners and Practice Manager enabling us to collaborate with numerous key partners including the CCG, Newcastle Hospital Trust, LMC and our Federation. This helps the practice to remain highly proactive in integrating the latest healthcare policy and digital systems, which makes every day challenging and rewarding.

Qualifications

Essential

  • General education to GCSE level A-C or equivalent Maths/English
  • Care Certificate or willing to work towards
  • NVQ level 2 Health and Social Care or willing to work towards

Desirable

  • Immunisation and Vaccine Accredited
  • Phlebotomy Training
