Job summary
CLCH Hertfordshire Adult Community Services - CLCH NHS Trust provides a range of Community Services, and the Hertfordshire Division is recruiting!
We are looking to recruit an experienced community clinician to support the locality in a duty clinician role.
This is a hybrid role consisting of office based clinical support and patient facing role where the need occurs.
Main duties of the job
To be responsible for the standards of clinical advice, assessment, promotion of self-management, care planning, implementation, and evaluation of individualised nursing care to patients living in the community.
Undertake patient telephone contacts relating to care needs, providing advice/ support.
Support with reallocation of work.
To support the nursing + Therapy team with clinical advice and support where necessary.
Where capacity dictates the post holder will be expected to undertake priority community nursing visits in patient homes.
To give skilled nursing care and advice, based on principles of best practice and clinical evidence. Including: end of life care, continence promotion and wound management, medication via syringe drivers and intravenous therapy, urinary catheterisation.
To ensure the safe handling of body fluids and contaminated sharps.
Act as the patient's advocate and to facilitate the patient's own choices with regard to nursing care, promoting independence and self-care, as appropriate.
Maintain accurate, comprehensive and contemporaneous records in accordance with Trust and NMC guidelines.
Liaise and escalate appropriate initiate referrals, to ensure adequate care and support to patients and carers.
To provide information and support to patients, relatives and other carers that promotes and optimises positive health.

Essential
- Registered Nurse (RN)
- NMC Registration
Desirable
- Evidence of other post-registration education and training
Experience
Essential
- Recent clinical nursing experience negotiating and liaising within a multidisciplinary team
- Working with people with complex health needs
- Supporting students and other learners e.g. HCAs teaching others in the practice environment
- Experience of undertaking general assessment by utilising evidence based care.
- Previous experience in the Nursing process, care planning and fluent in record keeping
- Experience of providing holistic care that involves the multidisciplinary team.
Desirable
- Post registration experience in a NHS community environment
- Leadership, support, induction, and teaching of other staff
- Evidence of participating in investigation/ management of complaints
Skills and Knowledge
Essential
- Knowledge of out of hospital care and the role of the Community Nurse
- Ability to listen effectively and ability to give constructive feedback to clients and colleagues.
- Knowledge of how Equal Opportunities can be implemented in practice
- Awareness of the management and use of data in the NHS setting
- Knowledge of risks involved in working in the community.
- Knowledge of management of long term condition e.g. Diabetes and an understanding of the concept of self- management.
- Literacy and numeracy skills
- Knowledge of reasonable response to service user needs and prioritising and dealing with those needs with a required urgency.
- Understanding of the concept of self- management
- Time management skills.
- Familiarity with electronic clinical systems including video consultations and digital photography, or willingness to learn.

- Car Driver, Hold a full, valid, UK driving licence and have access to a car to use for business purposes (unless you have a disability as defined by the Equality Act 2010)
