Job summary
Work to become embedded within local homeless hostels and services within Islington offering healthcare and clinical interventions as appropriate to people experiencing homelessness.
Main duties of the job
- To develop therapeutic alliances with people experiencing homelessness using the principle of trauma informed care, as a way forward to enable relationship building, assessment and treatment for people experiencing homelessness
- To undertake holistic physical and psychosocial health assessments at hostels and day centres for people experiencing homelessness, in order to identify unmet physical, mental health, addictions, safeguarding and social care needs. This includes the need for screening, health promotion and harm reduction work.
- To proactively identify immediate clinical risks when seeing individuals (e.g. related to possible infection, chest pain, abdominal pain, neurological concerns etc), and take appropriate action to enable the individual to access appropriate and timely care e.g. calling an ambulance, or taking senior adviceetc.
- To offer immediate wound care assessment and basic wound dressing as needed.
- To provide immediately necessary clinical / therapeutic primary care treatments if needed (based on best possible evidence) that will improve health outcomes for people experiencing homelessness where relevant e.g. wound dressings, prescribing etc and work to signpost to mainstream health services, and work to support patients into safe and effective treatment pathways where possible

Islington GP Federation (IGPF) is a growing organisation representing 31 practices; we have established ourselves as a leader in new ways of working, including running Islingtons extended access primary care services (I:HUB) as well as supporting the Islington Primary Care Networks (PCNs).
Our current range of services include the Extended Access Service, I:HUB, Community Ear, Nose and Throat (ENT), Integrated Community Gynaecology, practice-based pharmacists and a range of practice support mechanisms.
IGPF works very closely with a range of partners including the regional commissioning group, NHS England, Healthy London Partnership, Public Health, local hospitals such as UCLH, Whittington Health and the London Borough of Islington.
IGPF is the host organisation for the Primary Care Network (PCN) workforce and the Islington Training Hub and has been working for over two years to create training and development programmes that meet the needs of staff working in primary and community care settings.
IGPF runs four Islington GP practices, one on a GMS contract, two on APMS contracts and one caretaking contracts
Job responsibilities
Job Role: Inclusion Health Nurse Islington Health Outreach Team
Maternity Cover 6 month fixed term
Salary: £47576 - £50376 pro rata inclusive of London weighting
Hours: 16 hrs per week across 3 days
Tuesday and Wednesday 10am-4.30pm i.e 6.5hrs per day
Flexible half day - 3hrs option for remote work on half day

To provide screening e.g. blood borne viruses (BBV), diabetes, cervical cancer, where appropriate
To work within and consolidate integrated care and treatment pathways for individuals who are identified as being infected with BBVs (Hepatitis B, Hepatitis C and/or HIV), TB.
To undertake phlebotomy when required and safety can be maintained, ensuring adherence to identified pathways for blood sample labelling and collection.
To use blood results to support the assessment of the current health status of individuals, and refer for onward care as needed
To act as an advocate for people experiencing homelessness, supporting the individuals own choices regarding their care and promoting independence and self-care whilst at the same time being able to identify when a person is self neglecting
To document all clinical activity thoroughly including outlining any perceived risks and mitigations and in line with service guidelines
To manage own workload in such a way as to ensure there is enough time to undertake clinical work, reporting data and commitments and service meetings
To proactively develop and maintain close working relationships with and link in with homeless and inclusion health services, GPs Practices, Ambulance Service, mental health services, addictions services, and acute Trusts in order to enable positive joint working, and the seamless delivery of coordinated care to the homeless population
To attend case conferences with partner agencies for case managed individuals as necessary.

Qualifications
Essential
- First level registered nurse with a current PIN
- Trained in Phlebotomy
Desirable
- Registered cervical sample taker
- Qualification in Substance misuse
Experience
Essential
- Experience of working as a clinical practitioner in primary care setting
- Experience of delivering services in partnership with multiple partners with ability to independently represent the service at local level
- Experience of being an autonomous practitioner making independent decisions
Desirable
- Experience of working as a clinical practitioner in an inclusion health setting
- Possession of a nurse prescribing qualification
- Possessing of an advanced assessment qualification for chronic disease management
- Possession of an advanced clinical assessment qualification
- Experience of independently managing projects
