Job Purpose:

The Learning Disability and Autism Liaison Services ensures and consistently develops high quality care for people with a LD or who are Autistic across the Acute Care Trust, consistent with the principles and objectives contained in the Equality Duty Act (2010), Autism Act (2009), National Monitor framework Standards (Updated August 2015) and towards Learning Disability Improvement Standards for NHS trusts (NHSi, June 2018

The Learning Disability (LD) and Autism Liaison Services provides cover during the hours of 08.00 to 16.00 Monday to Sunday, but can sometimes arrange to offer support outside those hours by prior agreement. We aim to contact every patient (all ages) with a LD and/or Autism who is admitted to hospital within 24 hours, and work with the patient, carers, ward staff and community staff to ensure that this group of people are able to achieve the same health care outcomes as everybody else.

***Preference will be given to internal Trust staff, as well as 'Priority' and 'At Risk' status including NHS At Risk staff throughout Devon who are able to display recent relevant experience as dictated by the Person Specification.***

Main duties of the job

The Post Holder will:

Act as a positive role model in in the field of LD nursing through the provision of effective professional demonstration of competent, accountable clinical and most up to date practice.

Provide advice re assessments in relation to needs people with a LD or who are Autistic - such as use of the mental capacity assessment, safeguarding adults risk assessment, best interest decisions and deprivation of liberties assessments.

Ensure appropriate and timely input to care planning for individuals who have a LD or as needed people who are Autistic , mainly the assessment of reasonable adjustment needed.

Give specialist advice in the management of the interface between hospital departments/wards, specialist community services, adult social care services and primary care advising when appropriate risk assessments and MDT overview is required in complex cases.

Develop effective and collaborative working with healthcare and multi-agency colleagues in the management of people with LD/ or who are Autistic and additional common co-morbidities or other complex health/behavioural needs.

Incorporate the philosophy outlined in "Valuing People Now" and the principles embedded within the overarching structures as set within the Learning Disability Strategy/National Guidance, current Trust LD Standard Operation Procedure (SOP), Trust clinical governance framework and Equality Duty Act (2010)

Job responsibilities

Clinical Practice

Support the wards and departments across the Trust in their role to facilitate the meeting of individual healthcare needs for people with complex needs; input into risk assessments and input into plans of care to meet the needs of vulnerable individuals

Utilise information from other multi-agency risk registers such as child protection, domestic violence, public protection and Independent Safeguarding Authority (I.S.A.) in the safeguarding of vulnerable adults.

Liaise with appropriate agencies and members of the multidisciplinary team to ensure care in managed for at risk patients and to promote the safety and well-being of individuals

Assess and advise accordingly on issues surrounding the complex needs of people with LD/AS in relation to: Emergency and Elective Admissions, Discharge Planning, Access to investigations and diagnostics.

Work with the community specialist services to co-ordinate support with learning disability referrals that may require help to access acute services.

Act to raise awareness of Safeguarding Adults processes, ensuring concerns regarding vulnerable adults are responded to appropriately and making appropriate support Trust staff to make referrals within the current Safeguarding adult procedures

Identify and manage issues related to specific needs of vulnerable adults, raising awareness with medical and nursing staff managing patients in hospital

Share relevant information with appropriate partner organisations according to Information sharing protocols and governance, in order to manage risks to patients and ensure robust care planning to meet their needs and the safety of others.

Provide support and advice regarding best interest decisions and consent issues to individuals, their family/carer and the professionals involved in decision making about their care.

Attend and contribute to case conferences / clinical meetings and discharge planning meetings as appropriate, and to make referrals direct to social services / other agencies as required.

Provide health promotion advice to patients and their families/carers to support the maintenance of their health & wellbeing and their own safety.

Act as an advocate for users with learning disabilities or who are Autistic, ensuring the provision of appropriate information and support.

Maintain accurate, legally acceptable records, both in written format and on the trust computer system

Work clinically to delivery planned patient care in order to maintain clinical skills and credibility as a competent practitioner and leader.

Provide and receive sensitive or complex information and communicate at varyingly levels this to a range of individuals or professionals internal and externally to the Trust.

Provide advice on restraints in line with Trust policy to include physical and medicines restraints

Person Specification

Qualifications

Essential

  • oCurrent entry on NMC register RNMH/LD
  • oProfessional proven knowledge acquired through degree/diploma supplemented by specialist training or equivalent demonstrable experience

Desirable

  • oTeaching and/or mentoring qualification

KNOWLEDGE & EXPERIENCE

Essential

  • oDemonstrable experience of working with people with Learning Disabilities including those with autism to support in accessing an acute hospital setting.
  • oProven evidence of working within a range of learning disability healthcare/social care services for 2yrs or more
  • oProven evidence of ability to work across professional teams
  • oProven knowledge of Mental Capacity Act including Dols.
  • oWorking proven knowledge of community care act in terms of safe guarding adults processes
  • oProven knowledge of professional and national current issues in learning disability nursing and government legislation at specialist level.

Desirable

  • oUnderstanding of the wider NHS England, NICE guidance and local agendas relevant to the care for people with a LD and autism in accessing the acute hospital setting

Aptitude & Abilities

Essential

  • oProven ability to assess, plan, deliver and evaluate complex individualised and evidence based nursing care
  • oHigh level of communication skills with ability to demonstrate fluency, clarity and effectiveness at all levels.

Desirable

  • oWorking proven knowledge of conflict resolution and low level holds
