Main duties of the job

About the Role:

As a Locality Matron, you will be at the forefront of delivering compassionate, patient-centred care within the community. You will play a key role in managing and coordinating care for patients with complex needs, working closely with multidisciplinary teams to improve health outcomes.

The Locality Matron is also responsible for identification of those who are at risk and will provide pro active case management working with specialist nurses, medical colleagues, palliative care, intermediate care, social care, ACPs and Allied Health Professionals. The aim is to reduce hospital admissions and improve quality of life. Supporting patients with long-term conditions and complex health needs. Offering expert advice and guidance to patients and their families. Promoting health education and self-care within the community.Coordinating care between hospital, home, and community settings.

Job responsibilities

  • To clinically supervise and demonstrate specialist community healthcare knowledge and skills to develop the skill set of community nursing staff, students, patients and carers within a cluster of community teams.
  • To role model compassionate and inclusive leadership where team members are supported to work and develop clinical practice across all localities acting as joint supervisors to all Nurses where needed.
  • The post-holder will undertake clinical assessment, needs identification, planning, case management and co-ordination of care for individuals with highly complex needs and co-existing long-term conditions.
  • The post holder will contribute to and support delivery of the corporate agendas, ensuring the delivery of an efficient, high-quality service where nursing practice is evidence based and clinically effective.
  • The post holder will be collaborating as appropriate with the person, carer, Primary care networks and partner organisations. As a core member of the Management Team the Matron will ensure the achievement of performance targets, financial controls and professional leadership.

Person Specification

Qualifications

Essential

  • Registered Nurse
  • ENB 998 or equivalent teaching qualification
  • Evidence of current CPD
  • Degree or equivalent qualification specific to a clinical specialty
  • Evidence of further study/knowledge at masters level or equivalent

Desirable

  • Post registration qualification in an area of clinical specialty
  • Advanced Clinical Assessment qualification
  • Independent Prescribing qualification or an ability to achieve this within 2 years of appointment.
  • Degree or equivalent qualification specific to a clinical specialty
  • Management qualification

Skills and abilities

Essential

  • Excellent interpersonal and communication skills

Desirable

  • Ability to demonstrate emotional intelligence and leadership skills

Knowledge and skills

Essential

  • Knowledge of professional and NHS issues and policy and the ability to interpret policy at a local level.
  • Understanding of Information Governance and Confidentiality
  • Knowledge and understanding of application of relevant National Guidance e.g. NICE etc.
  • Knowledge of the NHS national targets and priorities
  • Understanding of the Trusts objectives and priorities and the ability to work across operational boundaries.
  • Evidence of current CPD
  • Recruitment and Selection Appraisal and other HR/People Management related training

Experience

Essential

  • Relevant leadership/management role in a large complex healthcare organisation
  • Significant post registration experience
  • Experience in leading and managing staff.

  • Experience in workforce planning and role redesign
  • Proven experience in managing and implementing change.
