Job summary

The Long Term Conditions (LTC) and Population Health (PH) Programme Manager will operationalise plans for LTCs and population health management (PHM).

Working in partnership, they will support decision-making and strategic planning across organisational boundaries, driving improvement.

The role involves supporting and leading transformation while ensuring value for money in planning, commissioning, and service quality. Project and change management expertise is vital to transform services and enhance access for patients, relatives, and carers. The post-holder will manage and implement the SNEE ICS PHM Programme within each Alliance to improve population outcomes. They will also hold a long-term condition programme area and apply PHM methodology to deliver focused and effective interventions.

The ICB Strategic Programmes team operates across the SNEE ICB footprint, covering Ipswich & East Suffolk, West Suffolk, and North East Essex alliances, embedding transformational developments across the Integrated Care System (ICS).

Candidates may come from clinical, care professional, public health, improvement, transformation, or analytics backgrounds. Strong communication and guidance skills are essential, bridging analytical staff and practitioners to deliver interventions. Candidates should have experience in using data to improve services, build relationships, and translate PHM intelligence into actionable methods while developing staff competencies in PHM.

Main duties of the job

  • SNEE ICB has a five-year joint forward plan the Long Term Conditions and Population Health Programme Manager will be responsible for operationalising the plan for Long term conditions and population health management. Effective project and change management expertise will be required to ensure transformation of services to improve access to services for patients, relatives and carers.
  • The Long Term Conditions and Population Health Programme Manager will support local projects and drive transformation as well as securing value for money in planning, commissioning and service quality.
  • Manage and implement the SNEE ICS Population Health Management Programme within each Alliance, developing and embedding the use of linked data, predictive analytics and partnership working in developing models of care and interventions to improve outcomes for our most vulnerable groups and improve health outcomes for the population as a whole.
  • Work with Analytical teams to analyse and interrogate data for all programs of work; work with transformation and alliance leads to prioritise; work with health and care practitioners, multi-disciplinary teams and voluntary sector workers to design and deliver the interventions for PHM and LTC area.
  • To ensure that resources of all kinds are used effectively and efficiently in the delivery of the LTC agenda and PHM programme.

The NHS Suffolk and North-East Essex Integrated Care Board (ICB) plans and buys healthcare services for our population. This function is commonly referred to as commissioning. Job responsibilities

The Strategic Programmes team works across the Ipswich & East Suffolk, West Suffolk and North East Essex alliances ensuring transformational developments are in place across the Integrated Care System (ICS) footprint. The Programme supports the delivery of NHS England National priorities in the long term plan, the SNEE Joint Forward plan and aligns the approaches undertaken in each ICS Alliance. This approach enables a more patient-centred focus to develop both providers and commissioner and their relationships with one another, across all care settings (community, primary, secondary, tertiary care). Health outcomes will improve through helping commissioners reduce unwarranted variation in services and encouraging innovation to deliver effective change to health care services through greater collaboration between providers and commissioners across different levels of the health care system.

The post holders may come from a variety of backgrounds, including clinical or care professional, public health, improvement, transformation or analytics. Their strength will be in enabling sound communication and guidance, bridging the gap between analytical staff and practitioners to deliver the interventions identified. The successful candidates will need to be experienced in using data, evidence and intelligence to improve services. They will be able to build relationships and translate PHM intelligence so staff can understand and apply methods and ideas, developing staff competencies in PHM.

Working in partnership with commissioners (including local government) and providers the Long Term Conditions and Population Health Programme Manager will support decision making and strategic planning across organisational boundaries acting as a vehicle for improvement for patients, carers, commissioners, providers, voluntary organizations and the public in order to:

Enable clinical and patient engagement: informing commissioning decisions

Define and drive quality improvement: operating across complex pathways of care

Coordinate and support commissioners and providers: identifying and reducing unwarranted variation, improving cohesion and ensuring sustainable services across pathways of care for staff and patients, both now and in the future.

Person Specification

Knowledge, Training and Experience

Essential

  • Highly developed specialist knowledge across range of procedures underpinned by theory and experience.
  • Knowledge of a range of specific project areas, acquired through post graduate diploma or equivalent experience or training, plus further specialist project management knowledge or experience to masters level equivalent.
  • Educated to masters level or equivalent of experience of working at a senior level in specialist area.
  • Extensive knowledge of specialist area, acquired through post-graduate diploma or equivalent experience or training plus further specialist knowledge or experience to masters level equivalent.
  • Evidence of post qualifying and continuing professional development.
  • Must understand the background to and aims of current healthcare policy and appreciate the implications of this on engagement.

  • Ability to make decisions autonomously, when required, on difficult issues, working to tight and often changing timescales.
