Job summary

This is an exciting opportunity for a MDT Care Coordinator to work with the Community Response Hubs of which there are three (Chester, Ellesmere Port and Rural). This role will involve working in partnership with different organisations to coordinate patient discharges from hospitals. You will lead on daily triage meetings and regular huddle meetings to support patients on their discharge.

This post will involve working across all three hubs.

Main duties of the job

to support safe discharges from hospitals

to collect relevant information and data within the Hubs

to manage the shared email inboxes to he hubs

general administration tasks, including loading patients and updating relevant electronic patient record systems.

managing daily triage and huddle meetings.

Qualifications

Essential

  • Good general education
  • Able to demonstrate a good command of English language, verbal and written.
  • Knowledge of Office Administration/Customer services acquired through training or experience to NVQ level or equivalent qualification

Knowledge and Expertise

Essential

  • Knowledge of Health and Social Care administration and processes.
  • Knowledge of Microsoft office applications, internet and email

Desirable

  • Knowledge/familiarity with medical terminology.

Experience

Essential

  • Able to work well in a team
  • Experience of working in an Office Environment
  • Experience/knowledge of issues of working with confidential information
  • Experience of working with healthcare professionals/previous experience in the NHS.
