This post has been designed following the success in securing the Lancashire County Council NHS Health Checks service. FCMS provides NHS Health Checks as a Community Outreach Provider to the eligible population of the Lancashire-12 districts.
Hours: 37 hours per week (Monday Friday)
Salary: £35,000 - £39,000 per annum - dependent on experience
Base: The post will be based at Newfield House, Blackpool. The post holder will also work externally building relationships with local stakeholders in the geographical areas where NHS Health Checks are delivered, in addition to being the primary interface and responsible for the relationship with Lancashire County Council
Main duties of the job
The NHS Health Checks Manager will be responsible for the management and operational delivery of the service, in addition to identifying and building relationships with stakeholders (including Lancashire County Council) to enable FCMS to secure appropriate venues in local communities for the delivery of health checks. Striving for the delivery of NHS Health Checks in line with contractual requirements alongside ensuring that our workforce is supported and cared for.
With a focus on logistical operational planning and delivery, in addition to relationship management/development, you will ensure that the Services meet the demands of commissioners, stakeholders including key employers and expectations of patients.

- Lancashire County Council
- GP Practices including priority practices
- Large employers
- Community, voluntary & faith groups (specifically within Priority Population Areas)
- ICB community engagement co-ordinators
- Lancashire County Council commissioned services relating to Stopping Smoking, Drug & Alcohol services, Healthy Weight Management and other lifestyle support organisations.
- Community sector meeting organisers and participants

Job responsibilities
Duties & Responsibilities
To provide compassionate, effective and visible leadership to the NHS Health Check team.
To ensure good people management practices and that effective & supportive team-working is felt across the NHS Health Check team
To support staff members to take personal responsibility and ensure they have the necessary autonomy to deliver an excellent service meeting contractual requirement
To be operationally responsible for the delivery of NHS Health Checks across the Lancashire 12 districts
To ensure provision of on call cover at weekends
To work with the Chief Operating Officer to ensure that NHS Health Checks meet (as a minimum) contractual obligations including KPIs and where these are not achieved, plans are created, actioned and monitored to ensure compliance
To ensure staff planning and rotas are based on good capacity and demand planning processes; consideration being given to complexities such as seasonality and historical data of clinic utilisation etc.
To work with the Business Intelligence Team to use operational dashboards to understand the operational activity of the department, that aids operational management and planning.
To work with the Business Intelligence Team to ensure KPI and Quality data and reports are produced on time and are an accurate reflection of the work undertaken. These form the basis of quarterly reporting to Lancashire County Council
To work closely with the Clinical Leadership Team to ensure compliance with external quality assurance processes (WEQAS)
To ensure we can evidence the positive impact of the work we do
To be responsible for managing the department budget
Act as FCMS primary point of contact in relation to requests received to support the provision of NHS Health Checks in the community
Respond to any requests to provide appropriate information, assisting decision making to lead to confirmed booking
Manage the Lancashire County Council rota and ensure that clinics are booked to enable us to meet contractual requirements
Create and maintain a directory of all stakeholders, contacts and venues for use in planning NHS Health Check clinics
Review clinic data to:
-Determine previously successful events in order that strong relationships can be made and events repeated in future
-Identify those events which have been unsuccessful in order that we de-prioritise/cease
-Proactively identify appropriate locations to carry out NHS Health Checks. This will include attendance at external community meetings to develop relationships and spending time in local communities to identify suitable options
-Analyse geographical data of events and venues in order that this is used to identify any gaps in service provision across all 12 localities, to ensure that all residents have equal opportunity to access a NHS Health Check in the community
-Analyse post event data to identify take up at events which will assist future event planning
Relationships
Identify and build relationships with stakeholders to enable FCMS to provide a successful Health Check programme for those who reside and work within Lancashire County Council. These include:

Work closely with the Commercial Support team to ensure that marketing support material is provided for community events and events are promoted via Social Media and NHS Health Checks website
This Job Description will be periodically reviewed in line with service development. This is an evolving role and therefore, these duties are not exhaustive. The role may change via discussion between the post-holder and COO. The individual in post will be expected to contribute towards that revision.
Education And In Service Training
Be fully conversant in all the clinical computer systems used within FCMS
Attend relevant study/induction days, seminars, courses etc, for individual development and for the benefit of the service.
Where necessary relevant training in the operation of new or unfamiliar equipment, software or procedures will be provided or arranged.
Confidentiality
In the course of your duties, you may have access to confidential information about patients, staff or health service business. On no account must such information be divulged to anyone who is not authorised to receive it. Confidentiality of information must be preserved at all times whether at or away from work. FCMS has in place a Whistle-blowers Policy for staff wishing to express concerns.
