Job summary
We have an excellent opportunity for a highly motivated individual to join our established Older Adult Community Mental Health Team on a fixed-term post until October 2026. Excellent communication and interpersonal skills are necessary for this post, with adaptability being essential as no two days are the same.
The team provides support for older adults experiencing mental health difficulties and the assessment and treatment for patients of all ages presenting with memory difficulties. As an assistant practitioner, you will be conducting memory assessments at our office base, within care homes and within people's homes within the locality, and subsequently taking these assessments through the process of diagnosis and treatment pathways where suitable. You will be monitoring the titrations of the anti-dementia medicines, understanding side effects and knowing when to escalate any concerns. Annual monitoring of these medications will also be completed via the telephone and face to face.
Liaising with various stakeholders will be a regular part of your job; this can include GP surgeries, carers and families, other teams within LPFT, and Adult Social Care. Regular joint working with other teams within LPFT, such as the Home Treatment Teams and the inpatient wards, requires some flexibility and shared responsibility.
Applicants will require a full UK driving licence. Full training will be provided for all responsibilities within this role.
Main duties of the job
To provide a high standard of care and comprehensive assessments to those experiencing mental health difficulties whilst working within the Trust's values
To monitor titrations of anti-dementia medications and complete annual reviews of anti-dementia medications used within the CMHT, and subsequently complete relevant documentation
Completing memory assessments at base, within people's homes, and within care homes in the Grantham/Sleaford locality, and completing relevant documentation.
Supporting individuals to develop independent living skills and live well at home
Supporting individuals in accessing the right services for their needs
To manage a caseload and service delivery to contribute towards the Trust's vision of keeping people well at home within the community
To work collaboratively and promote effective working relationships with members of the multidisciplinary team, ensuring effective and appropriate clinical decision-making, both within the CMHT and with external agencies.
To assess carers' and families needs and develop, implement and review programmes of support for carers and families.

Person Specification
Qualifications & Experience
Essential
- NVQ level 3 qualification or equivalent in a health/social care related subject.
- Training in mental health awareness or other training relevant to this post.
- Previous relevant experience working with people with mental health problems at a nursing assistant grade or equivalent within a health and social care setting.
- Experience in providing support for people in the community.
- Experience of working with people with mental health needs.
Desirable
- Willingness to engage in training provided by the division and professional training to support this role
- Previous experience within a dementia care setting or within dementia community services/ third sector providers.
