Hours: Monday - Friday 8.00am 18.00pm

Job responsibilities

Be primarily responsible for the rapid assessment and treatment of patients who have urgent and/or complex physical / mental health and social care.

Provide health care advice and education to team members, statutory, voluntary, and independent sector providers

Be responsible for health promotion, teaching and training of support workers, pre-registration students, patients, relatives, and carers

Ensure up to date written and electronic records and activity data are maintained in accordance with professional and trust standards

Prioritise caseload, with professional demands, considering the needs of other members of the team.

Person Specification

Experience

Essential

  • Community experience
  • Experience discharging planning of patients from hospitals
  • Experience of caseload management and prioritisation

Desirable

  • In addition to experience, all Qualified roles will be required to provide evidence of:
  • Relevant Qualifications
  • Registration with the relevant Registration Body

Qualifications

Essential

  • Current registration with HCPC
