Job summary

An exciting opportunity for an experienced Occupational Therapist to work within a specialist multi-disciplinary service has arisen with the CityCare Community Neurology team. The successful applicant would be responsible for the continuity and delivery of Occupational Therapy interventions for patients who have a diagnosed long term neurological condition, this will include conditions such as Multiple Sclerosis, Motor neurone disease, Huntingtons disease and Parkinsons Disease.

An understanding of neurological conditions, the physical, social and psychological impact of this long-term condition on the patient and family is desirable.

This is an opportunity to join a supportive and innovative team and to be part of a wider network of Occupational Therapists within Nottingham CityCare Partnership

The successful candidate will join the other band 6 OT in the team in providing:

Specialist Occupational Therapy Assessment and specialist interventions to support both patient, carers and wider team; promoting engagement with rehabilitation and self management. Working both in patients homes, care home and other community settings.

Informal / formal supervision of other members of the service i.e Assistant Practitioners.

Teaching and ongoing assessment of OT competencies of support staff and students.

Main duties of the job

As an occupational Therapist in the community neurology service you will have opportunity to:

  • Provide specialist assessment of patients functional abilities in their own home or other community setting, in order to establish treatment plans and person centred goal orientated plans, alongside an MDT including Physiotherapists, Specialist Nurses and support staff.
  • Deliver specialist treatment plans, including provision of specialist equipment, neuro splinting, fatigue management programmes, ADL related goals, falls management and postural management.
  • Engage in development and training within the team, through supervision with Band 7 OT, in-service training and other CityCare wide training opportunities.
  • To be involved in service development and transformation of services to enhance service delivery for patients and their carers.

Job responsibilities

Job Purpose

To be responsible for team related duties when requested by line manager or Team Manager.

To be professionally and legally responsible and accountable for all aspects of work, including management of patients within your care. To ensure a high standard of clinical work for patients under Occupational Therapy management, and support more junior staff to do likewise.

To undertake the comprehensive assessment of patients, including those with complex and/or chronic presentations using investigative and analytical skills and to formulate individualised management and treatment plans, using clinical reasoning and a wide range of treatment skills and options to devise specialist programmes of care.

To accept clinical responsibility for a designated caseload of patients and to organise this effectively with regard opt clinical priorities and use of time.

To demonstrate a commitment to evidence-based practice and make recommendations for change.

To be responsible for own continuing professional development- keeping abreast of new trends and developments, incorporating them into your work as necessary. Attend relevant conferences and internal/external training courses. Support more junior staff to do likewise.

To maintain a high level of Occupational Therapy standards and work to the Occupational Therapy Code of Professional Conduct.

To assist with development and participation in the teams audit projects, demonstrating use of evidence based projects and outcome measures, either individually or with senior therapists. To make recommendations for change.

To ensure the health and safety of staff and patients at all times in accordance with Health and safety legislation and Organisation Policies and Procedures. To adhere to all other Organisation Policies and Procedures.

To participate in identified data collection and statistics and to maintain clear, concise and timely records in line with service policy.

To provide clinical education, training, supervision and support to other staff and students within specialty area.

To be responsible for managing own specialist workload effectively within the clinical and professional policies of the Organisation.

Key Responsibilities

To work as part of a multi-professional , community neurology service providing support to adults with a range of neurological conditions.

To provide specialist Occupational Therapy assessment and treatment programmes for adults with a diagnosis of a neurological condition in a community setting.

To provide planned, coordinated rehabilitation to patients and their carers as part of the multidisciplinary team providing a service for adults with neurological diagnosis.

To identify health and social care needs and coordinate the input of other professionals within the team and other statutory and voluntary services.

To maintain records as an autonomous practitioner.

To educate, train and advise other health and social care workers, students and any other person involved in working with individual patients in the multi professional, and Occupational Therapy management of individuals needs , working in a variety of settings.

Organise own time including making appointments.

Experience

Essential

  • A level of post graduate experience demonstrating broad based experience which may include neurological rehabilitation, splinting and health care of older person that is pertinent to the area of work this post applied to including social care OT roles. Proven through attendance at courses and PRD.

  • Supervision of less experienced qualified staff and students/support workers.
