Job summary
The primary focus of the role is to work within the surgery as part of the General Practice Clinical team. The post holder will be working alongside a team of Allied Health Professionals, directly under the supervision of a GP. The post holder will work as part of a self-organising autonomous team to provide high quality and timely care to registered patients. The role will include holistic assessment, diagnosis, treatment and referral of patients within the individuals level of competence, supported by agreed protocols and agreement of the Partners. The role will require flexibility to ensure that there is adequate clinical cover at all times.
Main duties of the job
On a typical day, you will have your own patient appointment list and be required to manage your own workload and triage patients appropriately. As an autonomous Primary Care Paramedic, you are there to
Assist GP who is duty doctor that day
Undertake learning disability reviews for appropriate patients, resulting in the production of health action plans
Undertake mental health reviews for appropriate patients and updating their care plans
Assist with the production of advanced care plans for frail or end of life patients using the RESPECT tool.
See patients via either telephone or face to face contact, and treat patients making any necessary referrals to other members of the primary health care team.
Undertake consultations in the surgery or the community as an autonomous practitioner and using own clinical judgment to treat patients.
Make direct referrals to primary, secondary and social services within locally agreed pathways, guidance and protocols
Advise patients on general health care, minor ailments and long term conditions, with referral to other agencies as necessary
Assess social needs and complete referrals to the appropriate services or teams to keep patients independent .
Support the care of complex patients in their own homes (including nursing or residential accommodation) as part of a multidisciplinary

Undertake completion of care plans for patients on various at risk registers with associated read coding and template submission
To maintain accurate, contemporaneous healthcare records appropriate to consultation
Ensure evidence-based care is delivered at the highest standards, ensuring delivery of a high quality of patient care
Enhance own performance through Continuous Professional Development, imparting knowledge and behaviours to meet the needs of the service

To participate in the audit process, evaluation and implementing plans and practice change in order to meet patient need

Liaise across providers to ensure accurate and effective medication management Provide direct care to patients with long term conditions, minor ailments and other clinical areas within the scope of practice

Administration and Professional Responsibilities
Participate in the administrative and professional responsibilities of the practice team
Ensure accurate and legible notes of all consultations and treatments are recorded in the patients notes
Ensure the clinical computer system is kept up to date, with accurate details recorded and amended
Ensure accurate completion of all necessary documentation associated with patient health care, including templates and read-codes
Ensure collection and maintenance of statistical information required for regular and ad hoc reports and audit
Attend and participate in Practice/Clinical Meetings as required
Training and Personal Development
Training requirements will be monitored by yearly appraisal and will be in accordance with practice requirements. Personal development will be encouraged and supported by the practice. It is the individuals responsibility to remain up to date with recent developments
Participate in the education and training of students of all disciplines and the introduction of all members of the practice staff where appropriate
Maintain continued education by attending courses and study days as deemed useful or necessary for professional development

- As per both Government legislation and Practice policies ensure that all confidentiality, data protection and information governance policies and guidelines are followed and strictly adhered to. The Practice Manager will be required to report any infringement to the appropriate organisations immediately or as required.

- Relevant undergraduate degree and 3 years equivalent experience as a band 6 Paramedic.
