Job summary
The advanced nurse practitioner will be responsible for a number of clinical areas such as health promotion, chronic disease management, health prevention, as well as supporting the PCN leads and management team in the reviewing of clinical policy, practice and procedure with respect to the PCN enhanced access and hub services.
Apart from these clinical responsibilities, you will also work as an autonomous practitioner, responsible for the delivery of practice nursing services for the PCN and where required within individual PCN member GP surgeries, working as part of the PCN multidisciplinary team, delivering care within their scope of practice to the entitled patient population. You will be a healthcare professional NMC/GPC/HCPC registered with a license to practice, be an independent prescriber.
Main duties of the job
We are looking for an Advanced Clinical Practitioner Nurse who must;
have a masters degree level in the relevant area of expertise;
be working at a masters level aware or equivalent that encompasses the four pillars of clinical practice, leadership and management, education and research, with demonstration of core capabilities and area specific clinical competencies
have either graduated from a Centre for Advancing Practice accredited MSc advanced practice programme or completed the Centres ePortfolio (supported) Route; both pathways enable eligibility for an Advanced digital badge issued by the Centre, demonstrating recognised educational and experiential preparation in advanced practice
have current NMC registration.

North Lewisham Primary Care Network (NLPCN) is one of the largest PCNs in Lewisham and has a very diverse population made up of 9 GP Practices serving a population of around 80,000 patients.
Health inequality impacts our patients: North Lewisham has higher than average levels of deprivation and BAME populations and poorer health outcomes.
NLPCN is committed to tackling health inequality as a priority. We have developed a program of work to improve trust and engage with our local community, training our GP surgeries and staff to improve access for patients, improving our communication to patients about service changes and health promotion and improving the monitoring of those suffering from health inequality to drive our strategy.

The Role
We are looking to recruit to the post of Advanced Clinical Practitioner Nurse, to work within our Primary Care Network multidisciplinary healthcare team.
Main Role Responsibilities
The following are the core responsibilities. However, there may, on occasion, be a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels.
Clinical Care Delivery
Provide a first point of contact within the PCN/practice for patients presenting with undifferentiated, undiagnosed problems, making use of skills in history taking, physical examination, problem-solving and clinical decision making, to establish a diagnosis and management plan
Make professionally autonomous decisions for which you are accountable
Instigate necessary invasive and non-invasive diagnostic tests or investigation and interpret findings/reports
To prescribe safe, effective and appropriate medication as defined by current legislative framework
Provide safe, evidence-based, cost effective, individualised patient care
Offer a holistic service to patients and their families, developing where appropriate an on-going plan of care/support, with an emphasis on prevention and self-care
Promote health and well-being through the use of health promotion, health education, screening and therapeutic communication skills
Actively take a personalised care approach and population centred care approach to enable shared decision making with the presenting person
Refer patients directly to other services within or outside the PCN, and agencies as appropriate
Participate in developing and delivering specialist primary care services such as long-term disease management (diabetes, asthma, chronic obstructive pulmonary disease, coronary heart disease and hypertension).
Work with nursing, medical and health care assistant colleagues across the PCN to ensure that National Service Frameworks (e.g. Coronary Heart Disease/Older People/Diabetes/Mental Health) are being delivered
Work with the PCN multi-disciplinary teams as well as those within the PCN member GP practices and across the wider CIC, to promote integrated and seamless pathways of care
Contribute to the PCN achieving its quality targets to sustain the high standards of patient care and service delivery
Participate in identification of community health needs and develop patient/family-centred strategies to address them in liaison with appropriate community health professionals and agencies
Be aware of and contribute to, planning and delivery of practice-based commissioning

ANPs cannot provide care to pregnant women if this involves assessment of the pregnancy, unless they are also a practicing midwife meeting the NMC requirements of registration .
Professional role
Promote evidence-based practice through the use of the latest research-based guidelines and the development of practice-based research
Monitor the effectiveness of their own clinical practice through the quality assurance strategies such as the use of audit and peer review
Maintain their professional registration
Participate in continuing professional development opportunities to ensure that up-to-date evidence-based knowledge and competence in all aspects of the role is maintained
Work within the latest NMC Code of Professional Conduct
Record accurate consultation data in patients records in accordance with the latest NMC guidance and other pertinent standards
Keep up to date with pertinent health-related policy and work with the practice team to consider the impact and strategies for implementation
Pro-actively promote the role of the Advanced Nurse Practitioner within the PCN Community ward and externally to key stakeholders and agencies
Participate in multi-disciplinary protocol and patient group directions development
Identify appropriate opportunities to delegate both clinical and administrative tasks to more junior staff
Help the PCN operate in a cost effective manner
Identify and manage nursing care risks on a continuing basis
Participate in PCN meetings
Participate in audits and inspections as appropriate
Work closely with the clinical leads and administrative managers in the setting up and/or improving of systems for monitoring/measuring performance against the indicators for the domains and areas outlined in the current IIF
Work with the PCN member GP practices to assist them meet Clinical Governance and Quality Indicator targets
Ensure that all PCN policies are fully implemented
Work collaboratively with colleagues within and external to the PCN
Participate in audits and inspections as appropriate

Qualifications
Essential
- Current RGN Registration
- Membership of a professional body e.g. RCN
- Advanced Nurse Practitioner Degree or BSc in Emergency Care (or equivalent)
- Independent Non- Medical Prescriber Qualification (V300)
Desirable
- Advanced Nurse Practitioner MSc or working towards
