Job summary
Ranworth Primary Care Network have an exciting opportunity for a qualified clinical pharmacist to join our PCN on a full time, permanent basis, Monday to Friday. This role is based at Kennedy Way Medical Centre.
The post-holder will work in the proactive ambulatory team, working to provide expertise in clinical medicines management, medication reviews and repeat prescription authorisation.
Main duties of the job
- Review the on-going need for medicine of patients facing long-term conditions, monitoring needs and opportunities to support patients with their medicines ensuring they get the best use of their medicines (i.e. medicines optimisation).
- Undertake patient facing care home clinical medication reviews.
- Manage caseload of patients with common/minor/self-limiting ailments while working within a scope of practice and limits of competence. Signpost to community pharmacy and referring to GPs or other healthcare professionals where appropriate.
- Review the use of medicines most commonly associated with unplanned hospital admissions and readmissions through audit and individual patient reviews.
- Reconcile medicines following discharge from hospitals, intermediate care and into care homes.
- Identify and rectify unexplained changes and working with patients and community pharmacists to ensure patients receive the medicines they need post discharge.
- Provide a telephone help line for patients with questions, queries and concerns about their medicines.
- Manage the repeat prescribing reauthorisation process.

Person Specification
Qualifications
Essential
- GPhC independent prescriber qualification.
- GPhC registered pharmacist.
Desirable
- Minor ailments certificate.
- CPPE pathway completion preferred.
- Medicines management qualification.
- MUR and repeat dispensing certification.
Experience
Essential
- 2 years + working as a pharmacist (or equivalent).
- Experience in managing pharmacy services in primary care.
Desirable
- Broad knowledge of general practice.
