Job summary
APEX PCN is currently taking part in the NHSE national pilot to consider a proactive approach to meeting our patient needs. As part of this programme we are recruiting a fixed term Complex Care support team for our PCN population. This team will be assigned as a micro team to serve individual patient needs and take a consistent and personalised approach to patient care.
As part of this team we are looking to recruit a co-ordinator of the teams activity. This is a hugely exciting opportunity for us to work closely with our identified patients through a proactive MDT approach.
Main duties of the job
The successful candidate will ensure the smooth running of the team, co-ordinating appointments, managing patient records whilst providing excellent communication to both patients and health professionals.
The successful candidate would also be tasked to keep evidence of the work to support the delivery of the NHSE pilot. Support will be given from the PCN Manager and the Team Lead.

Job responsibilities
Provide comprehensive administrative support to the complex care team.
Manage and maintain accurate patient records using System One, ensuring data confidentiality and compliance with GDPR.
Coordinate and schedule patient appointments, referrals, and meetings.
Process and manage incoming and outgoing correspondence, including emails, letters, and tasks on System One
Answer telephone enquiries and provide information to patients, healthcare professionals, and other stakeholders in a professional and courteous manner.
Assist with the preparation of reports, presentations, and other documentation.
Maintain and organise office supplies and equipment.
Ensure adherence to organisational policies and procedures.
Utilise and understand medical terminology in daily tasks.
Assist with any other administrative tasks as required.
Prepare the agenda for a weekly MDT with the practices.
Collate evidence on the Teams achievements against the pilot KPIs
Person Specification
Qualifications
Essential
- GCSE grade A to C in English and Maths
Desirable
- Qualified to NVQ level 3
Experience
Essential
- Background working in a medical environment either primary or secondary care
- Experience using System One
- Good understanding of medical terminology
- Excellent organisational and time management skills
- Exceptional communication and interpersonal skills
- Proficiency in Microsoft Office Suite (Word, Excel, Outlook)
- Ability to work independently and as part of a team
- A strong commitment to patient confidentiality and data protection
- Ability to manage and prioritise workload effectively
Desirable
- Experience in a complex care setting
- Relevant administrative qualifications
- Knowledge of NHS procedures and policies
