Permanent - Part-time - 21 hours per week

The Co-ordinator will be jointly responsible for the effective co-ordination of the Placement Support Scheme.

The scheme is in partnership with the Integrated Care Board and takes referrals from Health and Social Care professionals, families and clients directly. The scheme will provide support and assistance for patients and families who are looking for a change of accommodation (including residential care) to a more suitable residence for their health and needs.

The purpose of the Placement Support Scheme is to help individuals/family/carers by providing a range of information and support, including transport (where appropriate) and signposting to other services to enable them to make informed choices about moving home, residential care and assisted living placement choices.

Main duties of the job

To meet with relevant stakeholders to discuss and promote the service

To accept and process referrals into the service from a wide range of sources

To speak with service users and their families over the telephone and in person to identify their needs and make service arrangements

To maintain regular contact with members on the scheme and ensure they have the resource and support they need

To build good relationships with care homes and referrers and maintain these

To build and constantly update a library of resources and information, relating to care home and assisted living venues and any supporting/signposting information

To undertake referrals and meet with and transport service users to care venues as agreed

Attend relevant meetings with senior managers and service commissioners as needed

Maintain appropriate records of work undertaken and produce written reports as required

Attend meetings of SCCCC as required

Ensure that all duties and functions are carried out in accordance with the organisation's regulations and policies

Carry out other duties and relevant tasks consistent with the responsibilities of the post and the wider Good Neighbour Scheme as directed by the Good Neighbour Scheme Operational Lead

SCCCC has over 50 years' experience in providing services that respond to the needs of vulnerable people in a practical and neighbourly way, complementing the care given by other service providers. SCCCC work in partnership with Sheffield Teaching Hospitals, Sheffield City Council and Sheffield Clinical Commissioning Group.

Accountability

The Co-ordinator reports to and will be supervised by the Good Neighbour Scheme Operational Lead.

Relationships

The Co-ordinator will be expected to develop good working relationships with all members of the SCCCC staff team and help maintain a mutually supportive environment.

Conditions of Service

The period of notice is four weeks on either side.

Holiday entitlement is 33 days (including statutory holidays - pro rata for part-time staff) taken at mutually convenient times. The holiday year runs from 1 January to 31 December.

It is important for the postholder to have a full driving licence and access to a car. Travel expenses will be paid in line with SCCCCs current policy. The postholder may at times need to drive a wheelchair accessible vehicle (training for this will be provided).

The postholder will work within the policies and procedures of Sheffield Churches Council for Community Care.

Person Specification

Experience

Essential

  • Minimum 2 years' experience in a relevant post or organisation
  • Experience working within community engagement and volunteering environment
  • Experience of contributing to a team environment
  • Evidence of recent relevant continuing professional development
  • Understanding of the needs of older people and their families
  • Ability to work effectively with a diverse range of individuals and organisations
  • Ability to communicate effectively both internally and externally to a wide variety of audiences and using a variety of methods suitable to different audiences
  • A proven ability to build robust relationships with key stakeholders to deliver excellent services, programmes or projects
  • Good telephone manner and the ability to deal sensitively with service users, family members and carers
  • Committed and highly self- motivated
  • The ability to work in a self-directed manner, and as part of a team
  • Computer literacy in dealing with standard MS office packages, including spreadsheets
  • Proven organisational skills and administrative ability, capable of meeting deadlines
  • Ability to work under pressure
  • Ability to adhere to an organisations policies, procedures and instructions
  • Ability to problem solve competently
  • Ability to relate well to volunteers
  • Able to act with integrity and take action if ethics, values or standards are compromised
  • Car driver/owner with full, clean UK driving licence and daily access to a vehicle essential
