Job summary
Northgate Medical Centre is looking for a dedicated and compassionate Practice Nurse to join our experienced nursing and healthcare team. We are a forward-thinking practice with a welcoming and supportive atmosphere. The position is part-time, 24 hours per week, salary dependent on experience. We would consider more or less hours for the right candidate. We actively promote and support continuing professional development for all of our clinical and non-clinical staff.
Main duties of the job
The successful applicant will be expected to develop their role and become a key member of the practice team. You will be actively involved in all aspects of GMS work. Administrative and clinical work will include full attendance at all practice meetings, delivery of QOF, enhanced services, manage long term conditions and chronic disease patients. Ideally, you will be a Practice Nurse with experience in a Primary Care environment. Our Practice delivers high quality care to our patients, and the successful candidate must have excellent communication, interpersonal and clinical skills to deliver effective and safe care.

Northgate Medical Centre is an inviting and friendly surgery, located in the centre of Chester. Our main focus is to provide the best possible care for our patients. We are situated in a fully modern purpose-built premises. Our practice list size is approximately 8800 patients. Our team consists of 3 GP Partners, 5 Salaried GPs, Practice Manager, 2 Deputy Practice Managers, 2 Advanced Nurse Practitioners, Nurse Manager, Practice Nurse, Nurse Associate, Healthcare Assistant, GP Assistant and an extremely experienced Administration and Reception Team. We are a Training Practice for GP Registrars, Medical Students, and Student Nurses.
Job responsibilities
- Implement and evaluate individual specialised treatment programs for chronic disease patients
- Respiratory Care including Peak Flow Readings & Spirometry
- Diabetes Care Management
- Cardiovascular Disease and Hypertension Management
- Cytology & Women's Health including Pill Checks
- Immunisations for Adults and Children
- Would Care including Complex Dressings, Compression & Doppler
- Travel Assessments and Vaccinations
- Ensure accurate records of all treatments and consultations are recorded on the clinical system, using SNOMED codes and templates as appropriate
- Support patients in the use of their prescribed medication or over the counter medication (within own scope of practice), reviewing annually as required
- Support the practice to ensure targets are met for QOF, LES & DES
- Collaborate with other agencies and services to help patient's wellbeing
- Ordering and checking of stock and vaccinations
- Support and work together as required on any research initiatives with the Practice
- Visit housebound patients at home when required
- Contribute to the education and training of students and participate in staff inductions to include awareness of infection control
- Provide education by attendance at courses and study days as considered relevant to your professional on-going education
- Any other duties delegated that are suitable to the post
Person Specification
Qualifications
Essential
- Experience of working in primary care
- Registered Nurse qualification with valid nursing registration
- Strong knowledge of QOF and Enhanced Services
- Excellent communication skills
- Routine Adult and Childhood Immunisations
- Respiratory Care including Peak Flow Readings and Spirometry
- Would Care, Complex Dressings, Compression and Doppler
- Diabetes Care Management
- Experience of working autonomously
- Understanding of primary care infection prevention and control
- measures
- Proficiency with general practice computer systems, Docman, EMIS
Desirable
- An understanding of GMS contract competency framework and
- current health contracts that impact on general practice
- Experience of undertaking audits and quality assurance
