Job summary

We operate a total triage model and are very dynamic in our approach to attending to patients needs. We use EMIS, Docman and AskMyGP. We have a very strong social Media outreach presence.

Our patients value the service we provide and we have 4.7 on Google with over 800 reviews.

The practice nurse must provide and maintain a high standard in general practice nursing which includes the diagnosis, monitoring and development of individual management plans, and agreeing these as appropriate with the patient and other health professionals. They will provide assessment, treatment, screening, health education services and advice to patients, while adhering to practice protocols, policies, and guidelines. The post-holder must provide and maintain the highest possible standards of nursing care for patients as well as provide nursing assistance to the doctors and other members of the team. They will do this independently and with nurse and GP colleagues. They will also lead on cervical screening as well as immunisations.

Main duties of the job

Maintain current registration with the NMC register and uphold the NMCs Code of Professional Conduct and their guidance at all times.

To continue professional development to maintain the highest level of competency required for your role through annual updates and any training as required.

Always adhere to practice and professional guidance, policies and protocols in relation to your role and the care of patients.

To always have patient care as the highest priority, using your expertise and clinical skills to accurately assess patients individual needs and manage their treatment using practice protocols, PGDs and appropriate guidelines.

Identify, manage, advise and support patients at risk of developing long term conditions, delivering opportunistic health promotion.

Understand and apply policy that supports the identification of vulnerable children and adults, being aware of statutory procedure and local guidance.

Actively participate in QoF and Enhanced Services activities.

Actively participate in audit activity and policy and protocol development for the evaluation and development of surgerys care and service to patients.

Produce accurate, contemporaneous, and complete records of patient consultations, consistent with the NMC and practice guidelines and protocols.

Support other members the team at all times both clinical and non-clinical, including mentoring and teaching where desired.

Take responsibility for their own work, as well as their learning and development at all times.

Mandatory (clinical):

All immunisations and vaccinations, including childhood immunisations.

Cervical smear screening

Asthma and COPD monitoring and management.

Diagnosis, monitoring and development of individual management plans; agreeing these as appropriate with the patient and other health professionals.

Understand the aims of management and the pharmacological treatments for chronic disease.

Initiate, perform and interpret diagnostic procedures as appropriate for chronic disease management.

Job responsibilities

Management and Administration

Provide and help to manage a caring, empathic and professional nursing and treatment service to the patients and practice team.

Liaise with other members of the surgery team and other agencies providing clinical care and support to patients within the local area.

Health & safety

The post-holder will implement and lead on a full range of promotion and management their own and others health and safety and infection control as defined in the practice Health & Safety policy, the practice Health & Safety manual, and the practice Infection Control policy and published procedures. This will include, but will not be limited to.

Using personal security systems within the workplace according to practice guidelines

Awareness of national standards of infection control and cleanliness and regulatory / contractual / professional requirements, and good practice guidelines

Responsible for the correct and safe management of the specimens process, including collection, labelling, handling, use of correct and clean containers, storage, and transport arrangements

Management and maintenance of Personal Protective Equipment (PPE) for the practice including provision, ordering, availability, and ongoing correct usage by staff

Responsible for hand hygiene across the practice

Ownership of infection control and clinically based patient care protocols, and implementation of those protocols across the practice.

Active observation of current working practices across the practice in relation to infection control, cleanliness, and related activities, ensuring that procedures are followed, and weaknesses / training needs are identified, escalating issues as appropriate.

Monitoring practice facilities and equipment in relation to infection control, ensuring that provision of hand cleansing facilities, wipes etc are sufficient to ensure a good clinical working environment. Lack of facilities to be escalated as appropriate.

Safe management of sharps procedures including training, use, storage and disposal.

Using appropriate infection control procedures, maintaining work areas in a tidy, clean and sterile, and safe way, free from hazards. Initiation of remedial / corrective action where needed or escalation to responsible management

Actively identifying, reporting, and correction of health and safety hazards and infection hazards immediately when recognised

Keeping own work areas and general / patient areas generally clean, sterile, identifying issues and hazards / risks in relation to other work areas within the business, and assuming responsibility in the maintenance of general standards of cleanliness across the business in consultation (where appropriate) with other sector managers.

Undertaking periodic infection control training (minimum twice annually)

Routine management of own team / team areas, and maintenance of workspace standards

Waste management including collection, handling, segregation, container management, storage and collection.

Spillage control procedures, management and training

Decontamination control procedures, management and training, and equipment maintenance

Maintenance of sterile environments

Demonstrate due regard for safeguarding and promoting the welfare of children.
