Job summary
Due to retirement within the team, we are seeking enthusiastic and reliable individuals to join our team as part-time practice nurse. You will be working as a team of practice nurses. This role primarily involves all aspects of practice nursing actively managing patients on chronic disease registers, wound care, immunissations / vaccination etc. The ideal candidate will have an interest in Diabetes, COPD, Asthma, CHD.
This role offers up to 30 hours per week.

Main duties of the job
All aspects of practice nursing, including theimplementation of processes for the effective management of patients with long-term conditions using evidence-based practice including care for patients.

Experience

- Experience of working in a healthcare setting

- Experience of working as a Practice Nurse, in a primary care environment.
- Experience of Chronic Disease Management- Asthma, COPD, Diabetes, CHD.
Desirable
- Experience of undertaking dementia and learning disability reviews.
- Independent nurse prescriber
