Job summary

Westcourt Medical Centre is a forward thinking and friendly training practice, with a growing list size that is currently around 13,000 patients. We are looking for an enthusiastic, motivated and qualified Nurse to join our Nursing and Health Care Assistant Team.

Main duties of the job

As an integral part of the clinical team, the post holder will work alongside our GP's, Paramedics, pharmacy and nursing team. The ideal candidate will have relevant experience in managing long term conditions such as COPD and Asthma, and additionally, experience of Child Immunisations, travel vaccinations and smears. This is an excellent opportunity for someone who is forward thinking, shares our commitment to delivering high quality patient care and who has the interest and energy to join us in further enhancing our nursing team. We would be keen to develop the successful candidate to all areas of practice nursing. Experience with TPP SystmOne is desirable but not essential.

Job responsibilities

1. Holistic approach to patient care

2. Develop, implement and embed health promotion and well-being programmes

3. Implement and evaluate individual treatment plans for chronic disease patients

4. Identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects on patients health

5. Provide routine nursing care to patients as required in accordance with clinical-based evidence, NICE and the NSF

6. Provide wound care (ulcer/Doppler etc.) to patients

7. Provide travel medicine services

8. Request pathology services as necessary

9. Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required

10. Maintain accurate clinical records in conjunction with extant legislation

11. Ensure read codes are used correctly and effectively

12. Maintain chronic disease registers

13. Chaperone patients where necessary

14. Prioritise health issues and intervene appropriately

15. Support the team in dealing with clinical emergencies

16. Recognise, assess and refer patients presenting with mental health needs

17. Implement vaccination programmes for adults and children

18. Support patients in the use of their prescribed medicines or over-the-counter medicines (within own scope of practice)

19. Liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients etc.)

20. Delegate clinical responsibilities appropriately (ensuring safe practice and that the task is within the scope of practice of the individual)

21. Support the clinical team with all safeguarding matters, in accordance with local and national policies

22. Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately

Deliver opportunistic health promotion where appropriate

Qualifications

Essential

  • Registered Nurse
  • Experience of chronic disease management
  • Wound care/removal of sutures and staples
  • ECGs
  • Venepuncture
  • New patient medicals
  • Chaperone procedure
  • Requesting pathology tests and processing the results, advising patients accordingly
  • Travel medicine
  • Diabetes
  • Hypertension
  • Asthma
  • Spirometry
  • CHD
  • Immunisations (routine, childhood and travel)
  • Womens health (Cervical cytology, contraception etc.)
  • Understands the importance of evidence-based practice
  • Broad knowledge of clinical governance
  • Ability to record accurate clinical notes
  • Ability to work within own scope of practice and to understand when to refer to GPs

Desirable

  • Experience of working as a practice nurse or community nurse
  • Knowledge of public health issues in the local area
  • Awareness of issues within the wider health arena
