Job summary
Morden Hall Medical Centre, located in SW London, is recruiting for a Practice Nurse to join our team initially as a maternity cover but with possibilities to work towards a permanent role. The role is to assist in providing general nursing care to our patients, as well as specific clinics relating to long term condition management, mentoring and leadership of our team of HCAs, as well as liaising with our other clinical teams of GPs and Pharmacists to ensure the safe and effective management of our patients health.
Previous experience within Primary Care, and/or Community or associated services will be essential.
The post will involve some late evening work up to 8 PM. However this would not be every day and will be covered as part of the team on a rotational shift pattern
Main duties of the job
Management of long term conditions - diabetes, asthma and COPD, coronary heart disease etc
Work closely with the practice doctors and nurses and, in particular, with the practice clinical data administrator to ensure all patients are being treated for and their care planned according to their needs
Planned and ad hoc care of older, disabled and housebound patients and patients over 75 years of age in their own homes, in residential and nursing homes and to all other patients who visit the practice for practice nursing care
Wound care, ulcer care, doppler
Removal of sutures
New patient medical examinations
ECGs, 24 hour ECGs
Monitoring of blood pressure
Venepuncture
Flu, pneumococcal immunisations, B12 and other injections
Baby and child immunisations
Baby clinic
Contraceptive checks
Infection control
Requesting pathology tests
Following agreed clinical protocols with referral to senior nurses or GPs as appropriate
Regular and timely liaison with appropriate practice doctors and nurses in relation to the health, care and treatment of individual patients
Supervision and continuing professional development of nursing team members including healthcare assistants

Job responsibilities
In addition to the previously notes responsibilities:
Cervical cytology (liquid based)
Travel clinic
Routine immunisations, childhood immunisations
Assisting GPs with minor surgery, implants and IUCD fittings
Chaperoning and assisting patients where appropriate who are being examined by another clinician
Maintaining and cleaning clinical equipment
Assist in maintaining doctor and nurse/treatment rooms, stocking and rotating items as required
Chaperoning and assisting patients who are being examined by another clinician.
Maintain general tidiness and cleanliness of nurse, treatment rooms
Maintain stock of waiting room leaflets
Ordering of health promotion literature
Ordering of stock
Ordering of vaccinations to maintain stock levels
A duty to advise doctors and other members of the nursing team of potential problems or errors within the range of assigned tasks
Any other delegated duties appropriate to the post
Administrative and professional responsibilities
Continuous liaison with all members of the practice team in relation to clinical, operational and strategic issues relating to the patient care provided by the practice.
Organise and co-ordinate the provision of nursing services for the practice to include the delegation of duties to other members of the practice nursing team as appropriate.
Participate in the administrative and professional responsibilities of the practice team.
Ensure accurate notes of all consultations and treatments are recorded in the patients notes on the clinical computer system as appropriate.
Ensure accurate completion of all necessary documentation associated with patient health care and registration with the practice.
Ensure collection and maintenance of statistical information required for regular and ad hoc reports.
Attend and participate in practice meetings as required.
Assist in the formulation of practice philosophy, strategy and policy and develop appropriate protocols.
Maintain a notice board in the waiting area designated for patient health care education.
