Job summary

An opportunity has arisen for an experienced forward thinking Practice Nurse to join our medical practice.

We are looking for an experienced Practice Nurse with excellent clinical and interpersonal skills to work 35 hours per week with flexibility to cover additional shifts and different days when necessary.

Main duties of the job

As a Practice Nurse you will be responsible for the delivery of general nursing services and chronic disease management to our patients.

Working collaboratively within our multidisciplinary team you will strive to meet the needs of our patients to improve outcomes, whilst maintaining the practices performance objectives.

Job summary:

The post holder is a caring, committed and experienced nurse who, acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. They will demonstrate safe, competent clinical decision-making and expert care, including assessment, diagnostic skills and prescribing, for patients within the general practice.

Job responsibilities:

Diagnosing and treating patients presenting with minor illness:

  • Triage and treat patients wishing to see a health care professional; provide appropriate care and/or make any necessary referrals to other members of the primary health care team.
  • Inform patients of test results and counsel them accordingly.
  • Ensure clinical practice is safe and effective and remains within boundaries of competence and to acknowledge limitations.
  • Advise patients on general health care and minor ailments with referral to other members of the primary and secondary health care team as necessary.

Management of Chronic Disease:

  • Contribute to chronic disease management by assessing, planning, monitoring and reviewing progress.
  • To support the clinical team in identifying and targeting patients for assessment and treatment.

Providing assessment, treatment, screening services and health education advice:

  • Chronic Diseases including asthma, COPD and diabetes
  • Health screening
  • Minor injuries/illnesses
  • HRT
  • Contraception/Depo injections/Pill check
  • Removal of IUCD / IUS
  • HVS
  • Pessary
  • Routine immunisations and vaccinations
  • Childhood immunisations
  • Routine injections
  • Zoladex implants
  • Travel advice / Travel f/u vaccine
  • Dressings
  • Removal of sutures / clips
  • Breath Test
  • New registration checks / general health advice
  • Dietary advice
  • Blood pressure checks
  • Urinalysis
  • ECG
  • Phlebotomy
  • Assisting with minor procedures in the treatment room

Person Specification

Qualifications

Essential

  • NMC registration required

Desirable

  • Additional specific nursing skills / qualifications - asthma / COPD /
  • diabetes

Experience

Essential

  • Experience of working within a nursing team

Desirable

  • Experience of working within a GP Practice
