We are excited to advertise this exciting opportunity which has now become available. This is a BRAND NEW Post due to Practice expansion and providing additional support to our existing Clinical team . This is a rare opportunity and would be unfortunate to miss. We are looking for an experienced Practice Nurse although experience in this field is desirable. The Practice will also consider investing in a Practice Nurse Career Start this will give the successful candidate the opportunity to engage in further training in the management of Long Term Chronic Conditions, Screening Programmes, engage on a one-to-one with the patients promoting Health & Well-Being awareness, Childhood Immunisations & National Vaccination Programmes We have an exceptional Nursing Team who work alongside a Team of Health Care Professionals. This team includes 4 GP's, Advanced Nurse Practitioner, Practice Nurse (Recently completed her Practice Nurse Career Start Training at Sunderland University). Health Care Assistant, Phlebotomist. There is HUGE opportunities for role progression. The successful candidate will be well supported in terms of training, & development

Main duties of the job

To manage patients with chronic disease, running specific clinic sessions and maintaining robust recall systems and accurate recall date entry

2.

Participate in national screening work such as cervical cytology

Carry out Cervical Smear Sampling and attend annual Cervical Cytology updates

3.

Provide Travel Health advice and administer appropriate vaccinations

4.

Undertake all Child Health Surveillance and Immunisation Vaccination Programmes

5.

Provide directly and support GPs & Nurse Practitioner with contraceptive services

6.

Provide ongoing support and have an empathy to patients/carers /parents' Palliative needs

7.

Act as chaperone when required

8.

Undertake health promotion to help educate patients

9.

Select, carry out and interpret results of specific tests

10.

Respiratory be willing to undertake further training to be ARTP accredited in Spirometry

Our WHOLE Attached & External Clinical Teams at present consists of 4 G.Ps, Advanced Nurse Practitioner, 2 x Practice Nurses, Health Care Assistant & Phlebotomist, Health Care Coach also working alongside our external staff which includes Practice Pharmacist, Pharmacist Technicians, Mental Health Nurse, Social prescribers, Health Coach.

Job Purpose:

To provide a high-quality care service based on the needs of the Practice population which includes treatment, screening, preventative care and patient education

Duties and Responsibilities

Clinical

General

To maintain own annual registration with the NMC and adhere to Code of Professional Conduct

To maintain a safe and therapeutic environment for all Practice staff, patients, carers and co-workers from other agencies. Ensure all statutory regulations (e.g. Health & Safety, COSHH, Infection Control etc are implemented and adhered to

To promote a professional approach at all times by attire and attitude to patients and staff both in and out of the Practice

To maintain own continuous professional development and ensure PDP kept up-to-date in line with the requirements of the Practice

Attend and participate in staff and Practice meetings and in-house clinical training sessions

Qualifications

Essential

RGN Qualification

Current NMC registration

Desirable but not essential training will be provided for the following

Accredited training in chronic disease management

Accredited training in anticoagulation management

Cervical cytology qualification

Experience

Desirable but not essential as training will be provided

Up-to-date training in childhood immunisations

2 years post-registration Band 5-6 - Desirable but not essential must HOLD the relevant RGN Training Certificate or equivalent

Qualifications

Essential

  • MUST have a GOOD educational Back Ground in main subjects C and above English, Maths, Science, Biology.

Desirable

  • MUST HOLD THE RELEVANT RGN NURSING QUALIFICATION OR EQUIVALENT
  • Must have experience in venipuncture
  • Vaccinations
  • Basic observations in the management of BP, Weight, Height, BMI
  • Experience in weight management advice & education signposting to weight management services
  • Management of LONG TERM Chronic Conditions Accredited (desirable but not essential as training will be given).
  • Manage Cervical Screening carrying out screening and follow up Accredited (desirable but not essential as training will be given)

Experience

  • Experience in a Primary Care or secondary Care Setting in a Nursing Role
  • Working with vulnerable and frailty patients.
