Job summary

We have a vacancy for an experienced Practice Nurse qualified in all chronic disease management. The vacancy is for 25-30 hours, spread over 3 or 4 days to suit. We would consider sharing the position between more than one applicant, so may suit a Practice Nurse/s that are looking for more permanent extra hours outside of their existing position.

We operate from two sites in rural Cheshire, Tattenhall and Farndon. Session times and days to be agreed with the successful applicant. Start date would be as soon as available. This is a permanent position with a highly successful and motivated team of staff to support you. Salary commensurate with experience.

Provide Practice nurse duties for our 8,700 patients. This to include annual reviews, ECG examinations and the full range of Practice Nurse duties on a shared basis with our two other nurses

Chronic disease management

Cytology

Wound care

Contraception

Hypertension management

Child and adult immunisations

Management of infection control

CQC compliance checks

Person Specification

Qualifications

Essential

  • NMC Registration
  • General Practice Nursing skills

Desirable

  • Warwick Diabetes, preferable

Experience

Essential

  • Experience of working in General Practice
