Job summary
Cardiff Community Resource Team is a community based multidisciplinary team consisting of Therapists, Nurses, Consultants and associated support staff. The team works to support people at home and either avoid hospital admission or facilitate early discharge, with rehabilitation being delivered within the person's own home. We work collaboratively with Cardiff Local Authority colleagues who provide domiciliary care when required. We work across the whole of the beautiful City of Cardiff to ensure support is delivered to people within their own communities and closer to home.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
Main duties of the job
The successful candidate will use previous experience and consolidation of practice, whilst working work under the direction of a registered practitioner, to organise and deliver community treatment and evidenced based falls programmes for identified patients in the locality.
The candidate will be required to work in patients' home in an unsupervised capacity whilst preparing and carrying out routine elements of patient assessment. Additionally, the successful candidate will have to plan and take responsibility for care/treatment of the community that they are accountable for.
Other requirements include monitoring patients and alerting therapists when there are unexpected changes, and modify approaches and activities within care/treatment using own clinical reasoning notify a registered practitioner accordingly, utilising evidenced based outcome measures where required.

- oNumerate and literate to GCSE level or equivalent

- oRecognised qualification NVQ level 3, Agored Cymru or equivalent.

- oBasic Food Hygiene
- oQualified or working towards a Level 4 Exercise Rehabilitation qualification
