If so, then North Bedfordshire Community Health Services are looking for a Rehabilitation Support workersto be part of the Transfer of Care Team. We are a 7 day service, 8am-9pm.
Main duties of the job
You will support service users remain at their place of residence, whether it be their home or a residential home by increasing their independence and ultimately their quality of life through rehabilitation of personal care, meal preparation of their bedtime routines. You will also support their physiotherapy goals, as well as those of daily activities of daily living.

Job responsibilities
Rehabilitation Support Workers play a key role in shaping and developing the service provided and so there will be opportunities to develop the role and service in a collaborative approach with Social care colelagues and the acute settings.
We recognise the need for development and career progression, so we offer clinical apprenticeship schemes, internal and external training courses and in service trainings to all staff.

- - NVQ 2 in Health and Social Care and/or significant care experience
- NVQ level 3

- Experience of ensuring that the values including Care, Compassion, Competence, Communication, Courage and Commitment are adhered to daily, and any matters of concern are raised with the relevant line manager or through the necessary processes within the Trust
- Have an understanding of the importance of quicker hospital discharges to medically optimise patients.
Knowledge and skills
Essential
- Sufficient IT skills, including use of Word and Outlook
- Regularly move and handle clients that may require moderate physical effort, in confined spaces and often unsuitable environments, using relevant practical experience, equipment and skills that enable optimum safety and best practice.
- Able to cope with the changing demands and unpredictable nature of the service
- Able to demonstrate a commitment to equality and diversity
- Able to support clients, carers and family during times of emotional distress
