We are looking for you to work as as part of the Aylesbury Rapid Response & Intermediate Care team supporting patients in their own homes , providing personal , social and rehabilitative care . The overall aims of the Rapid Response & Intermediate Care Team are to avoid inappropriate admission of patients into Hospital and facilitate timely discharge into the community.
The team is committed to ongoing education on the job and development opportunities are available within the organisation . Successful applicants who don't already have a Level 3 Qualification in Health and Social Care will be expected to work towards that in post.
Main duties of the job

Your main duties include:
- Providing general and specific rehabilitation focused care to adults, including personal care, exercise supervision, rehabilitation with domestic tasks and equipment delivery within competencies
- Being an integral member of the Rapid Response & Intermediate Care (RRIC) team, providing defined delivery of nursing, social and rehabilitative care to patients , working in collaboration with senior staff.
- Taking responsibility for designated elements of the patients care in patients' own homes.
- Being responsible for the completion of delegated non-clinical tasks to support the efficient functioning of the team.
Your main responsibility is to :
- Improve the care of patients in the community, so patients can remain safely in their own home for as long as possible
- Have an innovative approach, excellent communication skills and willingness to work with other members of the community teams to ensure high quality service delivery
- Willingness to further develop your clinical expertise in a range of areasA full UK driving licence and access to a car with Business insurance for work purposes

- Health and Social Care Level 2 or 3 Diploma or equivalent relevant patient facing experience and willingness to undertake Level 3 qualification.

- Previous caring experience of working in Health or Social Care environment.
