Job summary

We are looking for an enthusiastic salaried GP to join our Practice & be an essential part of a well staffed team & growth plans. Being part of a PCN, we have a full quota of support staff including ANP's, Care Navigators, Dieticians, Mental Health Practitioner, Podiatrist, Physiotherapists & a Social Prescriber. We are also supported by ICM team and District Nurses and Hospice

Main duties of the job

Key responsibilities & duties: Surgery consultations, triaging calls as appropriate, occasional home visiting & administration associated with patient care & to run duty sessions. Work as a team member with all relevant providers of primary and community care services within the Practice. Ensure appropriate records are maintained at all times regarding patient care & that relevant team members are kept fully informed regarding patients seen on behalf of the Practice. Ensure that the Practice achieves the highest QOF outcomes. Developing care plans for health in consultation with patients & in line with current practice disease management protocols. Support the proactive care agenda by participating in care planning & multi-disciplinary meetings. Attend team & healthcare meetings when required & represent NMC Partnership at external meetings.

About us

We are based in Romford, Essex, one site practice, 10,000 patients, EMIS web system used. We have high QOF achievements. Our team includes: GP partners, ANP's, highly skilled Nurses, HCA, Practice Manager, Secretaries, Administration staff, Receptionists, PCN staff. We also have an active PPG and ICM Team.

There is off-street parking available at the site .

Job responsibilities

We are looking for a salaried GP to cover 6 sessions, Monday - Wednesday. Duties will include: Surgery consultations, triaging calls as appropriate, occasional home visiting & administration associated with patient care & to run duty sessions. Work as a team member with all relevant providers of primary and community care services within the Practice. Ensure appropriate records are maintained at all times regarding patient care & that relevant team members are kept fully informed regarding patients seen on behalf of the Practice. Qualifications

Essential

  • Fully GP trained with all essential skills

Desirable

  • Gypsi skills or GP trainer or wanting to be a trainer
