Are you looking to develop your skills and knowledge to build a career? Frimley Health Foundation Trust has an exciting vacancy for a Band 6 Senior Community Dietitian on a part time basis of 22.5 hours per week. Flexible work options can be considered.
The responsibilities within this role include delivering a dietetic service to housebound patients and Care Home residents at risk of malnutrition in the Frimley South area.
The post holder will work closely with the wider relevant multi-disciplinary teams and provide support to Medicine Optimisation team.
You will lead on various projects such as Appropriate prescribing of Sip Feeds and Quality Improvement projects in care homes.

Main duties of the job
- To provide individualised nutrition assessments and review for patients requiring oral nutrition support in care homes and those who are housebound across Frimley South Place
- To be a source of expertise for FHFT and Frimley ICB on issues relating to oral nutrition support in the community
- To support team lead and Frimley ICB medicines optimisation team in ensuring appropriate use of oral nutritional supplements in the community to treat malnutrition
- To educate, train and disseminate information to health and social care professionals including clinicians, nurses, allied health professionals, dietetic colleagues, care home staff and students about the dietetic management of malnutrition in the community
- To undertake regular audit and service development projects
