Job summary
An exciting new opportunity has arisen for an experienced registered nurse to work full time within the Self-Management and Ambulatory Care Service.
This would include using advanced knowledge of managing complex wounds, Line Care, Catheter Care, Wound Care, Injections, suture removal.
You will be responsible ensuring the delivery of the clinical pathways appropriate to the neighbourhood as well as:
- Oversee the triage function within each service
- Managing a caseload of complex patients as an autonomous practitioner within this specialist area
- Support staff with clinical training needs and development, such as caseload reviews and in service training.
- Provide day to day line management to clinical staff and undertake 1-1s/appraisals/clinical supervision
- Managing resources, capacity and demand functions within the services, which involves complex clinical decision making#
- Support and oversee student placements within the services
- Undertake audits and data analysis as required
- Continuous engagement and networking with wider services and organisations, to promote integrated working and knowledge sharing.

Main duties of the job
The post holder will maintain clinical credibility demonstrated through undertaking clinical practice on a weekly basis, as agreed with their Line Manager. This will include practicing as an autonomous clinician to support service delivery, responding to social, scientific, clinical, and ethical issues in line with the values and agreed model within the service. and patient demands.
Self-Management is achieved by increasing the knowledge, skills and confidence a person has in managing their own health and care by putting in place interventions such as: peer support, self-management educationand health coaching.Leeds Community Healthcare recognises the benefit to patients in managing their own health and long-term conditions across all populations of Leeds. This is championed by our team of Self-Management Facilitators.
The Integrated Clinics provide care for patients in a clinic setting with over 16 sites in Leeds. A team of registered clinicians and healthcare professionals, provide assessment, treatment, and intervention for patients, for a range of complex health needs. We work in partnership with Primary Care Partners, to deliver care needs for patients within Leeds.
This is a citywide service, and the postholder will be expected to travel across all areas of Leeds as required on a regular basis.
