An innovative role has arisen for a senior community Physiotherapist to join our established integrated intermediate care team. The team provide an urgent community response service to avoid unnecessary hospital admissions and a virtual ward to support patients to remain at home wherever possible and facilitate safe hospital discharge.

This is a 24 hour a day 7 days a week service operating 365 days a year therefore the post holder will be expected to work shift patterns to cover the service including Early, Lates or Long Days and have access to own transport.

Main duties of the job
The post holder will:
- Provide a personalised intermediate care service to adults in their own home within Airedale, Wharfedale and Craven areas. This also includes providing therapy support to patients in Castleberg Hospital.
- Perform Physiotherapy assessment of patients with a diverse range of presentations and complicated physical and psychological conditions, to provide a diagnosis, develop and deliver an individualised treatment programme in conjunction with the rest of the team.
- Provide a key role in working in partnership with medical colleagues, multidisciplinary teams (MDT) and carers to set realistic objectives and to help to facilitate goal and discharge planning to enable safe discharge from our service.
- Work to support the team in its delivery of interventions, to skill share with other professionals to support the full breadth of the service (competency training is offered to support this skill sharing method of delivering intervention).
In pursuing these duties, the post holder will ensure compliance with the NHS Constitution and be familiar with the NHS England Guidance 'Understanding the new NHS'.

Qualifications
Essential
- Degree or equivalent pre - registration award in Physiotherapy
- UK registration with the Health Care Professions Council
Desirable
- Relevant qualifications in a related subject and / or evidence of post registration training and development
- Member of the Chartered Society of Physiotherapy
Experience

- Physiotherapy assessment and treatment skills
